摘要
目的:探讨穿孔素(perforin,PFP)在IgA肾病(IgAN)肾组织中的表达及意义。方法:采用Elivision的方法检测IgAN肾活检组织中PFP的表达。结果:IgAN患者PFP(17/41,占42.8%)在肾间质表达。41例患者中轻度肾间质病变23例,PFP阳性9例(9/23),中度肾间质病变12例,PFP阳性6例(6/12),2例重度病变,PFP均阳性(2/2)。轻、中、重肾间质PFP的表达阳性率差异无显著性。PFP的表达与患者血清肌酐水平,24小时尿蛋白定量无相关性。结论:PFP在IgAN肾组织内主要表达于肾间质内,提示PFP在IgAN的肾间质损害中可能起一定的作用。
Objective：To study the expression of perforin （i,e,pore-forming protein,PFP）in renal tissue of IgA nephropathy（IgAN）and its significance,Methods：The expression of perforin in renal biopsy specimens of IgAN was detected by Elivision.Results：In the renal mesenchyma, the positive rate of perforin expression in IgAN patients was 42.8%.23 of 41 IgAN patients had mild tubulointersitial lesion,9 of 23 patients with mild tubulointersitial lesion had perforin expression.6 of 12 patients with moderate tubulointersitial lesion had perforin expre^sion.2 of 2 patients with serious tubulointersitial lesion had perforin expression.There was no significant difference in the positive rate of perforin expression among the mild,moderate and serious tubulointersitial lesions.There was no significant association among the perforin expression,serum creatinine and 24-hour urinary protein,Conelusion：Perforin expression was significant in the renal mesenchyma of IgAN.Perforin might play a certain role in renal interstitial lesion of IgAN.
