目的探讨帕金森病(PD)常规MRI检查白质高信号(WMH)与临床的关系。方法分析64例PD患者和45例健康老年人对照组常规MRI检查WMH、脑室周高信号(PVH)、深部白质高信号(DWMH)和年龄、病程、分级的关系。结果PD患者WMH发生率较对照组增高,但无显著性差异(P>0.05),有WMH的PD患者较无WMH的患者年龄更大(P<0.01),病情更严重(P<0.01)但病程无显著性差异(P>0.05);PD组DWMH发生率较对照组略高,但无显著性差异(P>0.05);有DWMH的PD患者较无DWMH的患者年龄更大(P<0.01),但病程、病情严重程度无显著性差异(P>0.05);PD组PVH发生率较对照组增高,有显著性差异(P<0.05),有PVH的PD患者较无PVH的患者年龄更大(P<0.01),病情更严重(P<0.01),但病程无显著性差异(P>0.05);PD患者和对照组的DWMH分级无显著性差异(P>0.05),但PVH分级有显著性差异(P<0.05)。结论PVH与PD的一些临床特征有关,PVH可能会影响病情的严重程度。
Objective To explore the correlation between white matter hyperintensities (WMH) and the clinical features of Parkinson's disease (PD) when examined by magnetic resonance imaging (MRI). Methods The correlation between WMH, periventricular hyperintensities (PVH), deep white matter hyperintense signals (DWMH) in MRI and age, disease duration, distribution in 64 patients with PD and 45 age-matched healthy controls was studied. Results The frequency of WMH was higher in patients with PD, but no significant difference compared with controls ( P〉0.05). The patients with WMH were significantly older ( P〈0.01) and more severity (P〈0.01) than those without WMH, but disease duration was no significant difference (P〉0.05). The frequency of DWMH was higher in patients with PD, but no significant difference compared with controls ( P 〉0.05). The patients with DWMH were significantly older ( P 〈 0. 01) than those without DWMH, but disease duration and severity were no significant difference ( P 〉0.05). The frequency of PVH was higher in patients with PD and having a significant difference compared with con-trols ( P 〈0.05). The patients with PVH were significantly older (P〈0.01) and more severity than those without PVH, but disease duration was no significant difference (P〉0.05). The distribution of DWMH was not significantly different between patients with PD and controls (P〉0.05), but distribution of PVH was significantly different between patients with PD and controls (P〈0.05). Conclusion PVH may influence some clinical features of PD characterized by a greater disease severity.
