摘要
目的探讨PTEN和p16基因在肝细胞性肝癌(HCC)中的表达及其意义。方法采用免疫组化技术S-P法检测PTEN蛋白及p16蛋白在43例HCC及其相应癌旁组织中的表达。结果PTEN蛋白在HCC组织中的阴性表达率明显高于在癌旁组织中的阴性表达率(P<0.01);PTEN蛋白表达阴性率与HCC的分化程度、临床分期呈负相关,与AFP值及有无转移、HBV感染相关,与p16蛋白缺失率呈正相关,而与患者年龄、性别、肿瘤直径无关。比较PTEN阴性表达和阳性表达的HCC患者的术后1,3,5年生存率,差异在统计学上有显著性意义。结论PTEN失活部分参与了HCC的发生、发展进程。PTEN基因、p16基因的失活及HBV感染可能在HCC的发生、发展中存在协同关系。PTEN蛋白表达的检测可为HCC患者术后的预后判断提供参考。
Objective To explore and discuss the potential correlation between the expression of PTEN and the expressopm of p16 in primary human hepatcoellular carcinoma and its significance. Methods The specimens from 43 cases of HCC were collected to detect PTEN and p16 expression with immunohistochemical staining (S - P) in formalin fixed paraffin-embedded sectioons. Results The rates of negative staining of PTEN protein in tumor tissures were higher than that in the correlated adjacent fissures, there were higher negative rates of PTEN expression in poorly differentiated HCC specimens and lower negative rate in well differentiated HCC spocimens. Reduced PTEN expression was correlated with advanced tumor stages, HBV infection, metastasis of HCC, higher serum AFP levels and reduced expression of p16, but did not show correlation to other parameters measured in this study, including sex, age and size of tumors. Furthermore, compared with negative expression of FFEN protein, these patients with positive expression had higher survival rates after surgery. Condusion The inacfifivafion of PTEN takes part in the carcinogenesis and the developing process of some HCC. There maybe a coonrdination among inactivation of PTEN gene, p16 gene and infection of HBV. The expression of PTEN gene can serve as one of indictators for judging the prognosis of HCC after surgery.
