2型糖尿病患者定量超声骨密度含量的变化及相关因素的研究

摘要 目的探讨2型糖尿病人定量超声骨量的变化规律及其影响因素。方法测定118例2 型糖尿病患者的超声传导速度(SOS),按性别、年龄、尿白蛋白排泄率(UAER)进行分组,采用Student-t检验进行组间比较;同时与性别、年龄、病程、体重指数(BMI)等作多因素分析。结果(1)男性SOS值及其标准差均显著高于女性(P<0.01),男、女组SOS值随UAER的增加而降低,正常白蛋白尿组(NA)与大量白蛋白尿组(MA)之间差异有显著性(P<0.05);(2)男性病人SOS改变仅与UAER(对数值)呈负相关(P<0.05),女性病人与年龄呈负相关(P<0.01);(3)女性糖尿病病人骨质疏松发病率显著高于男性患者(P<0.05)。结论2型糖尿病女性患者定量超声骨量下降较男性明显;年龄与UAER分别是糖尿病女性和男性骨质疏松发病的相关因素。 ObjectiveThe changes of quantitative ultrasonic bone density (QUS) and the possible related factors were investigated in type 2 diabetic patients. MethodsThe speeds of sound (SOS) were measured in 118 cases with type 2 diabetes mellitus. The student-t test was implemented to study the changes of quantitative ultrasonic bone with different sex, age, and UAER patients. The relationship between the SOS and sex, age, BMI and UARE were analyses of multivariate in patients. Results(l) SOS and its SD in male diabetic group were significantly higher than that in male group. SOS of male and female groups was lower in macroalbuminuria group (MA) than that in normal albuminuria (NA) (P〈0. 05). (2) SOS in male diabetic group was inversely related to UARE (P〈0. 05), and that in female diabetic group was related to age (P〈0. 01). (3) The rates of osteoporosis in female diabetic patients was significantly higher than that in male group (P〈0. 05). Conclusion QUS in female diabetic group was significantly lower than that in male group. Age and UARE were the related factors in the pathogenesis and progress of osteoporosis in type 2 diabetes patients.
