摘要
目的探讨人类白细胞抗原(HLA)-DQB1等位基因与儿童十二指肠溃疡(DU)和幽门螺杆菌(H.pylori)感染的遗传关联性。方法采用非同位素标记的聚合酶链反应-序列特异性寡核苷酸探针(PCR-SSO)杂交的方法,对上海地区汉族健康儿童80例、DU患儿58例的HLA-DQB1等位基因进行分型;并同时检测DU患儿H.pylori感染情况。结果在DU患儿中,HLA-DQB1×05031等位基因频率明显高于正常健康儿童(分别为:6.02%、0.63%,P<0.05,RR=9),而在H.pylori阳性和H.pylori阴性DU组之间差异无显著性。结论HLA-DQB1×05031与DU呈正相关,DU患儿与正常对照组儿童之间存在着遗传学的差异;虽然H.pylori感染是DU的重要致病因素,但HLA-DQB1×05031并不是通过H.pylori感染而影响DU的遗传易感性。
Objectives To explore the immunogenetic association of HLA-DQB1 with duodenal ulcer (DU)and Helicobacter pylori (H. pylori)infection. Methods Fifty-eight DU patients and 80 healthy controls were examined for HLA-DQB1 genotypes. HLA-DQB1 typing was carried out with polymerase chain reaction-sequence specific oligonucleotide(PCR-SSO) . The incidence of H. pylori infection was tested in DU patients. Results The HLA-DQB1 × 05031 allele frequency was significantly increased in the DU than that in control group(6.02%vs 0.63%, P〈0.05,RR = 9). However, those who were H. pylori-positive didn't have an increased risk for duodenal ulcer than their H. pylori-negative control. Conclusions Genetic difference may appear in the HLA-DQB1 locus between DU and healthy controls. The HLA-DQB1 × 05031 is a susceptibility gene for duodenal ulcer,but this relationship is not associated with H. pylori infection.
