摘要
10例接受右心漂浮导管检查的慢性阻塞性肺疾病(慢阻肺)患者一次性舌下含服钾通道开放剂尼可地尔粉剂15mg,5min后肺动脉平均压由(3.15±0.42)kPa下降到(2.75±0.37)kPa,肺血管阻力由(56.94±8.90)kPa·s/L降至(44.65±6.39)kPa·s/L下降幅度分别为(12.69±1.86)%和(19.66±3.16)%,30min后仍维持在此水平。心输出量用药前为(3.50±0.28)L/min,用药后为(3.95±0.27)L/min,增加(17.60±4.88)%。体动脉血压和体循环阻力无显著变化。用药后血氧饱和度和氧运输值增加,动脉血氧分压、二氧化碳分压和氢离子浓度变化不明显。结果表明尼可地尔能降低慢阻肺患者肺动脉压和肺循环阻力,且有一定选择性,可改善慢阻肺患者心功能,并增强氧运输功能。
We observed the changes of pulmonary hemodynamics by doing right heart float-assisted catheter in ten patients with chronic obstructive pulmonary disease (COPD)who sucked sublingually single dose (15 mg) of Nicorandil powder. After five minutes,mean pulmonary artery pressure (mPAP)lowered from (3.13±0.42)kPa to (2. 75±0.33)kPa, Pulmonary vascular resistance (PVR)declined from (56. 94±8. 90) kPa. s·L(-1)to(44. 65±6.39) kPa·s·L(-1), their decreasing percentage being (12.69±1. 86)%and (19. 66±3. 16)% respectively. Within thirty minutes when the experiment went through,mPAP and PVR remained in the low level. Cardial output increased from (3. 50±0.28) L·min(-1) to (3. 95±0. 27) L·min(-1) by (17.60±4.88)%. Systemic blood pressure and systemic vascular resistance were unchanged. Aterial blood oxygen saturation and oxygen transport increased remarkably, But the effects on PaO2, PaCO2 and CH were not statistically significant. The results suggest that Nicorandil has some selective effects of decreasing mPAP and PVR in patients with CDPD, and may improve the patients' cardiac function and increase oxygen transport.
