摘要
目的:研究豚鼠胆囊切除术后胃肠移行性复合波(MMC)是否与胆酸池大小有关.方法:60只豚鼠分别给予正常饮食8wk(A组、B组),10g/L胆固醇饮食8wk(C组、D组),喂养结束后,评估胆囊动力并将电极植入胃肠道;其中B组、D组同时行胆囊切除术,术后4wk记录MMC并测量胆酸池大小.结果:胆囊切除后,正常与胆结石豚鼠MMCI,II,III期时限与总周期平行缩短而波幅升高,但无显著性差异.胆结石豚鼠比正常豚鼠胆酸池减小(P<0.05),并且手术后再轻微下降;而胆酸、粪胆酸轻微升高,其程度与正常豚鼠手术后的变化一致.然而,这些变化没有统计学差异.结论:在正常与胆结石豚鼠,胆囊切除术没有引起MMC与胆酸池大小改变,但MMC运动与胆酸池密切相关,因此,胆囊切除术后综合征不是胃肠动力障碍所致.
AEVI: To investigate whether the bile acid pool size after cholecystectomy is correlated to the gastrointestinal migrating myoelectric complex (MMC) in guinea pigs. METHODS: Gallbladder motilities were assessed before cholecystectomy and interdigestive gastrointestinal motilities were continuously monitored using bipolar electrodes in conscious guinea pigs before and 4 weeks after surgery in standard diet group and high cholesterol diet (cholesterol gallstone) group. Total bile acid pool sizes were measured by isotope dilution method. RESULTS: After cholecystecto- my, there were parallel falls in duration of phase Ⅰ, Ⅱ, Ⅲ and MMC cycle duration but increase in amplitude in the guinea pigs with normal gallbladder function, and in the guinea pigs with cholesterol stones, but no significant difference was found ( P 〉0.05). The bile acid pool was definitely small in the gallbladder stone (GS) guinea pigs compared with that in normal guinea pigs ( P 〈 0.05) and became slightly smaller after cholecystectomy. The bile acid in gallbladder bile and fecal bile acid slightly increased in GS guinea pigs after cholecystectomy, similar to those in guinea pigs with normal gallbladder function. CONCLUSION: Cholecystectomy produces a similar but not insignificant change in the bile acid pool size in guinea pigs with normal gallbladder function and in the guinea pigs with cholesterol stones. MMC is associated to the enterohepatic circulation of bile acids. Gastrointestinal dyskinesia is not the result postcholecystectomy syndrome.
