摘要
目的评价反映肾脏滤过功能损伤的血清生化标志物半胱氨酸蛋白酶抑制剂C(Cys C)的诊断效能.方法计算机检索1993~2003年之间的Medline、Embase和中国生物医学文献数据库,按照纳入和排除标准收集相关文献31篇,进行Meta分析.结果 Cys C的综合比数比(42.31)和综合受试者工作特征曲线(SROC曲线)下面积(0.935)较大.根据诊断标准和测定方法进行分组分析,2组间分析差异均无显著性,前者组内分析只有99mTc-DTPA差异有显著性(P<0.05),后者组内分析颗粒增强透射免疫比浊法(PETIA)和颗粒增强散射免疫比浊法(PENIA)差异都有显著性(P<0.05).结论 Cys C对肾脏滤过功能损伤的诊断效能较大,建议采用PETIA为Cys C的测定方法,选择51Cr-EDTA为参考标准,采用前瞻性队列设计.
Objective To evaluate diagnostic value of cystatin C for indicating glomerular filtration function injury. Methods Articles that are involved in those biomedical markers are indexed by computer in medline, embasc, and chinese biomedical literature database from 1993 to 2003 ,then thirty one articles are selected according to including and excluding criteria and evaluated systematically. Results Systematic odds ratios (42.31 ) of cystatin C and areas under summary receiver operator characteristic ( SROC ) curve(0.935 ) are biger. Sellected articles were subgrouped according to diagnostic standards and analytically techniques. Inter-group analyses of cystatin C were all not statistically different, the former intra-group analysis of 99m Tc-DTPA was statistically different ( P 〈 0.05 ), the latter intra-gmup analysis of partical-enhanced turbidimetric immunoassay(PETIA) and partical-enhanced nephelometric immunoassay( PENIA ) were statistically different (P 〈 0.05 ). Conclusions In systematic review, cystatin C is considered as the obviously useful marker indicating glomerular filtration function injury, PETIA is commended to measure cystatin C, ^51Cr-EDTA is selected for intermediary to measure glomerular filtration rate in investigation of cystatin C, and feedforward cohort is recommended to apply.
