摘要
目的:回顾颈椎前路减压、植骨方法在临床中的应用情况,进而寻求理想的前路减压、植骨方法,促进颈椎病患者的神经功能改善和植骨融合率的提高。资料来源:应用计算机检索Medline数据库1990-01/2004-12期间的关于颈椎前路减压方法及植骨材料的文章,检索词“Advanceofanteriordecompression,bonetransplantationfinomaterialsofceruicle”,限定文章语言种类为“English”。同时计算机检索中国期刊全文数据库1995-01/2004-12期间的颈椎前路减压方法及植骨材料的文章,检索词“颈椎前路减压、植骨材料”,限定文章语言种类为中文。资料选择:对资料进行初审,选取包括上述颈椎前路减压方法及植骨材料的文献,然后筛初非随机实验的研究,对剩余的文献开始查找全文,以是随机对照实验作为纳入标准,排除重复性研究文献。资料提炼:共收集到85篇关于颈椎前路减压方法及植骨材料的随机和非随机实验,18个试验符合纳入标准,排除67篇。资料综合:18个试验包括运用不同方法进行颈椎前路减压,使用不同植骨材料进行椎体间融合,并且对手术治疗效果进行分析。结论:颈椎前路手术以成为脊髓型颈椎病治疗的主要方法,颈椎前路减压方法及植骨材料发展迅速,大量研究发现环锯减压、圆柱状自体植骨块植入技术,对于椎间盘切除患者,临床骨融合率最高。
OBJECTIVE: To review the application of anterior decompression of cervical vertebra and bone transplantation in clinics so as to look for ideal method of anterior decompression and bone transplantation to promote amelioration of neural function and improve confluent rate of bone transplantation of patients with cervical syndrome. DATA SOURCES: Using the key words “Advance of anterior decompression, bone transplantation fino materials of ceruicle”, we retrieved the Medline database of all articles related to anterior decompression and bone transplantation in English language between January 1990 and December 2004. Meanwhile, we retrieved the Chinese Journals Full-text database with the same key words in Chinese between January 1995 and December 2004. STUDY SELECTION: All articles were selected firstly, and those related to anterior decompression and bone transplantation were obtained. Articles without random research were excluded. The rest articles of full text were selected on the basis of random control study and duplicated investigation was excluded. DATA EXTRACTION: Totally 85 articles related with anterior decompression and bone transplantation with or without random trail were selected. Among them, 18 articles were coincident with the inclusion criteria, and other 67 were excluded. DATA SYNTHESIS: The 18 trials included various anterior decompressions and materials of bone transplantation for confluence of vertebral body, the operative effect was analyzed at the same time. CONCLUSION: The operation of anterior approach is the main method for the treatment of cervical spondylotic myelopathy. Anterior decompression and bone transplantation develop rapidly, and a large number of researches suggest that it is the highest confluent rate for patients with intervertebral disc resection by the technique of trephine decompression and cast bone auto-transplantation.
