明胶/壳聚糖创面敷料对新鲜创面收缩性的影响 被引量:7

摘要 目的:探讨一种透明型明胶/壳聚糖敷料对新鲜创面的收缩性的影响。方法:实验于2004-03/12在唐山市工人医院美容整形科完成。将壳聚糖溶解于适宜的稀酸溶液中,加入溶解于水的明胶,并加入一种适宜的增塑剂,充分搅拌混合,流涎法铺膜。制成明胶/壳聚糖(g/g)分别为5∶10(盐酸溶解),10∶10(分为盐酸和乳酸溶解),15∶7(盐酸溶解),明胶/壳聚糖/甘油(g/g)分别为10∶10∶4(分为盐酸和乳酸溶解),10∶10∶6(盐酸溶解)7种膜。在24只成年大鼠的背部作一新鲜创面,切至皮下组织层,面积为4cm×4cm。分别将不同制法的7种明胶/壳聚糖膜与创面自然贴附,不借助其他敷料覆盖其上。以凡士林纱布贴附为对照。每种敷料贴3只鼠创面处。黏附能力的分级在置于创面后即刻,3,4,7d测量评估:黏附面积<50%为较差,50%~69%为一般,70%~90%为良好,>90%为优秀。观察不同制法的明胶/壳聚糖膜用于新鲜创面的收缩性及抑制新鲜创面的收缩性,抑制收缩率=[创面长(mm)×创面宽(mm)/原长(mm)×原宽(mm)]×100%。结果:24只大鼠所有创面均进入结果分析。①不同制法的明胶/壳聚糖膜对新鲜创面的黏附性:0~4d明胶/壳聚糖膜黏附面积均达到优秀(>90%)和良好(75%)。7d,伤口边缘的黏附逐渐减弱,渗出物和肉芽组织的生长及动物的活动导致膜的周边与伤口分离。②不同制法的明胶/壳聚糖膜用于新鲜创面的收缩性:4h收缩为38.0~40.5mm。③不同制法的明胶/壳聚糖膜抑制新鲜创面的收缩性:7d收缩率,凡士林纱布为69.2%,乳酸溶解明胶/壳聚糖膜为85.2%,盐酸溶解明胶/壳聚糖膜为100.1%。明胶/壳聚糖膜抑制新鲜创面的收缩性效果好于凡士林纱布。结论:明胶/壳聚糖膜用于新鲜创面时,具有延缓收缩的能力。有些明胶/壳聚糖膜有轻微的收缩,是由于甘油的复脱水作用促进了收缩,用乳酸溶解壳聚糖比用盐酸溶解的膜更易导致收缩,故 AIM: To investigate shrinking effect of a kind of transparent gelatin/ chitosan wound dressing on newly incised wounds. METHODS: The experiment was conducted in the Department of Cosmetology and Plastic Surgery, Tangshan Worker's Hospital from March to December 2004. Chitosan was dissolved in the preferable dilute acid, and then gelatin which could dissolve in water and a kind of plasticizer were added into this solution. After the solution was stirred evenly, film was overed by means of drooling. Then the solution was made into the 7 kinds of films at the gelatin/chitosan (g/g) ratio of 5 to 10 (solved by hydrochloric acid), 10 to 10 (solved by hydrochloric acid and lactic acid respectively), 15 to 7 (solved by hydrochloric acid), and at the gelatin/ chitosan/glycerol ratio of 10 to 10 to 4 (solved by hydrochloric acid and lactic acid respectively), 10 to 10 to 6 (solved by hydrochloric acid). A fresh wound at 4 cm×4 cm were made by excising subcutaneous tissues on each back of 24 adult rats. The films were not pasted to the wounds by any means other than natural adhesion, and no other dressing was placed on it. The paste of Vaseline was used as the control. Each kind of dressing was used in 3 rats. The degree of adhesion was assessed immediately after placement on the wound, at 3, 4 and 7 days based on the following criteria: adhesion area less than 50% as be poor, between 50% and 69% as be fair, between 70% and 90% as be good, and more than 90% as be excellent. The shrinkage of different kinds of gelatin/chitosan film used in fresh wounds and the anti-shrinkage of fresh wounds were calculated according to the formula: anti-shrinkage rate=[shrunk length (mm) of wound (mm) ×shrunk width of wound(mm)/initial length(mm)×initial width(mm)]×100%. RESULTS: All the wounds of 24 rats were analyzed in the result.①The adhesion degree of different kinds of gelatin/chitosan film on wounds: The adhesion of gelatin/chitosan film was excellent (more than 90%)
