摘要
目的总结再次肝移植的临床经验。方法回顾分析我中心再次肝移植病人临床资料,并结合文献进行讨论。结果共有8例病人接受了再次肝移植术,再次肝移植率为3·64%,再次移植原因中慢性排异反应2例、胆道缺血性病变5例、肝动脉栓塞1例。术后5例恢复顺利痊愈出院,已分别存活8、7、4、3、1个月,3例死亡,1例死于感染性休克,1例死于多器官功能衰竭,1例死于颅内出血。结论再次肝移植能有效挽救移植肝失功病人的生命,再移植指征的掌握、手术时机的选择、手术技巧的提高和围手术期的正确处理是提高再次移植成功率的关键。
Objective To summarize the experience in liver retransplantation. Methods The clinical data of 8 patients received liver retransplantation in our hospital were retrospectively analyzed and relevant literature reviewed. Results For the 8 patients, 2 received liver transplantation for chronic rejection, 5 for ischemic biliary lesion and 1 for hepatic arterial thrombosis. Five of them were cured and 3 died because of serious infection, failure of major organs and bleeding in brain. Conclusions Liver retransplantation can effectively save the patients with graft failure. Proper indication, optimal operating time, improvement of operative skills and appropriate treatment during the perioperative period are very important for promoting rate of success of liver retransplantation.
