摘要
目的研究巨细胞病毒(HCMV)感染症与不孕、异位妊娠的关系,探讨HCMV抗体检测对盆腔炎性粘连的预测价值。方法分析142例患者的腹腔镜术前HCMV抗体检测结果和术中情况,比较不孕、异位妊娠组和卵巢囊肿组HCMV抗体阳性率的差异,以病例对照研究方法分析巨细胞病毒感染症与盆腔粘连的关系,研究盆腔炎性粘连程度与血清HCMV抗体阳性率的关系。结果不孕与异位妊娠组HCMV抗体阳性率(51.25%)明显高于卵巢囊肿组(14.29%,P<0.001)。盆腔粘连发生率HCMV抗体阳性病例(93.88%)明显高于HCMV抗体阴性病例(31.18%,P<0.001,随盆腔炎性粘连程度加重,血清HCMV IgG抗体阳性率增加(P<0.001),盆腔炎性粘连程度与血清HCMV IgG抗体阳性率之间关系较密切。随粘连程度增加,IgA抗体阳性率增加不显著(P=0.023),盆腔粘连炎性程度与血清HCMV IgA抗体阳性率之间关系不密切。结论异位妊娠及不孕的发生与HCMV感染有较密切的联系。感染巨细胞病毒较易引起输卵管异常、盆腔粘连。在预测盆腔炎性粘连程度时,IgG抗体较之IgA抗体更有价值。
Objective To investigate the relation between human cytomegalovirus (HCMV) infection and infertility and ectopic pregnancy. And to discuss the imprortance of detection of anti-HCMV antibody in prognosis of inflammatory conglutimation in pelvic cavity. Methods The anti-HCMV antibody was detected in 142 patients sorted to 2 groups, ectopic pregnance, or ovarian cyst group. The relation between inflammatory conglutination in pelvic cavity and the positive frequencies of anti-HCMV antibody detection were analysed by case-control study. Results The positive frequencies of anti- HCMV antibody in infertility and ectopic pregnance group (51.25%) were significantly higher than that of ovarian cyst group (14.29%, P 〈0.001). The inflammatory conglutination in pelvic cavity were significantly more frequent in positive anti-HCMV antibody group (93.88%) than in negative group (31.18%, P 〈0.001, OR= 33.839). The positive frequency of anti-HCMV IgG, not that of IgA, increased significantly with the degree of inflammatory conglutination in pelvic cavity ( P 〈 0.001, with Pearson contingency coefficient of 0.613). Conclusions HCMC infection, which could cause uterine tubes disfunction and conglutination in pelvic cavity, have close relation to infertility and ectopic pregnance. Anti-HCMV IgG was more important in predicting the degree of inflammatory conglutination.
