摘要
目的探讨宫颈癌根治术中行卵巢移位术术后卵巢的功能状况.方法 1999年1月至2003年12月江门市新会区人民医院等2家医院收治43例年龄26~40岁宫颈癌ⅠB~ⅡA期患者,其中22例在宫颈癌根治术中行卵巢移位术为观察组,21例行传统的宫颈癌根治术为对照组. 术后应用血FSH、LH、E2 及Kuppermann评分进行卵巢功能测定,随访1~4年.结果观察组术后4年内卵巢功能基本正常,对照组术后1个月卵巢功能丧失.两组记录生存质量的改良Kuppermann评分比较差异有非常显著性意义(P<0.01),而复发率和存活率比较差异无显著性意义(P>0.05).结论宫颈癌患者在宫颈癌根治术时行卵巢移位术,可保留卵巢功能.
Objective To investigate the function of transposited ovaries following radical hysterectomy in young women with early stage of cervical cancer. Methods Totaly 43 young women (aged from 26 -40 years old) with early stage of cervical cancer (FIGO ⅠB or ⅡA) were studied from Jan 1999 to Dec 2003 in Xinhui People's Hospital,Jiangmen city and the Second Municipal Hospital of Guangzhou. Ovarian transposition was performed at the time of radical hysterectomy and pelvic lymphadenectomy in 22 cases, which was designed as the research group; only radical hysterectomy and pelvic lymphadenectomy were done without ovarian transposition in the other 21 cases, as the control group. Ovarian function was evaluated by serum FSH, LH, E2 and Kuppermann score system in all of the cases during postoperative 1-4 years follow-up. Results While the patients in control group became climacteric in one month after the operation, normal ovarian function was preserved in the patients of the research group within 1- 4 years postoperative follow-up. There was a statistically significant difference between the research group and the control group regarding serum E2, FSH, LH level and to Kuppermann score of the patients ( all P 〈 0. 01 ); also, there was not a statistically significant difference between the two groups regarding recurrence rate and survival rate (all P 〉0. 05). Condusion Our results demonstrate that ovarian transposition protects ovarian function in young patients undergoing radical hysterectomy and pelvic lymphadenectomy for early stage of cervical cancer.
