摘要
目的分析左侧隐匿性斜行旁道的心脏电生理特征,快速、准确地标测消融靶点,提高射频消融的成功率。方法对38例阵发性室上性心动过速患者均在窦性心律及诱发室上速进行心脏电生理检查,确定左侧隐匿性旁道,标测心室侧消融点图。结果4例隐匿性斜行旁道的患者,均经股动脉逆行法二尖瓣下消融成功,但较非斜行旁道费时。结论斜行旁道,尤其是与二尖瓣环角度小、行程长的斜行旁道,以左室起搏信号S到冠状窦电极上最早逆行A波最短间隔处为靶点,可提高射频消融成功率。
Objective To analyze the characteristics of intracardiac electrogram of left concealed oblique atrioventricular accessory pathways and find way for ablation target quickly and accurately in order to improving success possibilities. Methods Thirty-nine cases were under some intracardiac electrophysiological study at sinus rhythm and paroxysmal supraventricular tachycardia. Then ensured whether they were left concealed oblique atrioventricular accessory pathways. Then marked up ablation target graphs of ventricle side. Results Four cases of left concealed oblique atrioventricular accessory pathways were successfully ablated finally. But all of them took much time than others.Conclusions To the oblique atrioventricular accessory pathways of long and small angle with mitral valve annulus, the point of the shortest distance from left ventricular pace signal S to earhest ventricular activation(EVA) was marked as ablation target can improve probabihty of success.
