急性白血病患儿多药耐药基因表达的临床意义 被引量:2

摘要 目的探讨急性白血病(AL)患儿多药耐药(MDR)相关基因(mdr1)、多药耐药相关蛋白(MRP)、谷胱甘肽-S-转移酶(GSTπ)、肺耐药相关蛋白(LRP)、拓扑异构酶Ⅱ(TOPⅡα)、TOPⅡβ的表达及临床意义。方法用逆转录-聚合酶链反应(RT-PCR)技术检测139例AL患儿上述6种MDR相关基因的mRNA表达水平。结果1)难治组前5种基因表达水平与敏感组相比有非常显著差异(P<0.01)。2)复发14例患儿初治时LRP、TOPⅡα表达水平与复发后存在非常显著差异。3)6种基因表达水平与年龄及白细胞计数无相关性。4)经多元相关分析发现所有病例mdr1与MRP,mdr1与GST,πmdr1与LRP,GSTπ与LRP,GSTπ与MRP,LRP与MRP间呈正相关(P<0.01),TOPⅡα与mdr1,MRP,GSTπ呈负相关(P<0.05)。结论mdr1、MRP、GSTπ、LRP、TOPⅡα是独立于年龄及白细胞之外的反应儿童AL复发难治的预后因素,LRP、TOPⅡα与继发耐药有关,不同耐药基因间存在相关性。 Objective To investigate the expression of gencs nultiple drug resistanle(mdr1 ). multidrug resistance associated protoin(MRP), -glutathione transterase (GSTπ). lung resistance relined protein (LRP) and topoisonmerase (TOP) Ⅱ α/β involved in multidrug resistance (MDR) and clinical significance. Methods Fxpression of the six genes above mentioned were measured in 139 acute leukemia patients by RT-PCR methods. Results 1. The expression levels of mdr1. MRP,GSTπ, LRP and TOPⅡα/β in refractory group were very different from those in sensitive group (P〈0.01). 2. Fourteen relapse children were compared between newly diagnosed and relapse, and there were significant ditterences in the expression ot LRP,TOP Ⅱα 3. The expression of these genes uneorrelated with white blood cell count and age of chilrden. 4. Significant correlations were obeyed between mdrl and MRP, mdrl and GSTπ. LRP and mdrl. GSTπ and LRP, MRP and GSTπ LRP and MRP, TOPⅡα and mdrl, TOP Ⅱ= mind MRP, TOPⅡα and GSTπ in all children. Conclusions The expression of mdrl. MRP. GSTπ, LRP. and TOP Ⅱ α are independent unfavorable prognostic factors of relapse and refractory in children with acute leukemia. LRP and TOP Ⅱα have the relationship with the secondary muhidrug resistance. There are, correlations between different drug resistance genes.
