摘要
目的:探讨脉压在脑卒中、冠心病、高血压患者中的分布情况及与上述疾病间的关系。方法:检测2003-03/07在安阳钢铁公司社区居住者1789人,其中血压正常者998例,脑卒中68例,冠心病258例,高血压465例的血压及脉压。采用多因素Logistic回归分析不同脉压水平人群患脑卒中,冠心病和高血压的危险性。结果:1789名被调查者全部进入结果分析。①脑卒中组、冠心病组、高血压组、血压正常组脉压均值分别为(58±21),(57±20),(60±19),(41±9)mmHg,F=223.39,P<0.0001,多重比较结果显示脑卒中、冠心病、高血压组脉压显著高于血压正常组;随着脉压增宽,脑卒中、冠心病、高血压的患病率也呈上升趋势。②在调整了性别、年龄、体质量指数、吸烟及饮酒后,以脉压<45mmHg的人群为基线组,经Logistic多因素回归分析,脉压为45~59,60~74和≥75mmHg组人群,脑卒中的患病危险分别是基线组的1.87,2.47,3.79倍。冠心病的患病危险是基线组的1.42,2.99,3.20倍。高血压的患病危险是基线组的1.63,1.92倍。将收缩压和舒张压纳入影响因素中再分析,仍然显示上述疾病的患病危险随脉压的增宽而增加。结论:①脑卒中、冠心病、高血压的患病率随脉压增宽而增高。②脉压增宽是导致心血管疾病发病率增高的危险因素。
AIM: To explore the distribution of pulse pressure in the patients with stroke, coronary heart disease (CHD)or hypertension and the relationship between pulse pressure and all these diseases. METHODS: The blood pressure and pulse pressure of the inhabitants from the community of Anyang Iron & Steel Co. Ltd. were detected from March to July 2003, including 998 people with normal blood pressure, 68 patients with strike, 258 patients with CHD and 465 patients with hypertension. The risk of getting stroke, CHD and hypertension in people with different pulse pressure was analyzed by the multiple factors Logistic regression analysis. RESULTS: 1789 testees were all involved in the result analysis. ① The pulse pressure value in the stroke group, CHD group, hypertension group and normal blood pressure group was [(58±21), (57±20), (60±19), (41±9)mm Hg,F=223.39, P 〈 0.000 1], respectively. After multiple comparisons, it was showed that the pulse pressure of the people in the stroke, CHD and hypertension group was significantly higher than that in the normal blood pressure group; As the pulse pressure became wide, the prevalence rates of stroke, CHD and hypertension rose up. ② After the adjustment of sex, age, body mass index, smoking and drinking, the people of 〈45 mm Hg pulse pressure was considered as base line group. The risk rate of getting stroke of people in 45-59, 60-74 and ≥75 mm Hg group was 1.87, 2.47 and 3.79 times of that in the base line group, respectively. The risk rate of getting CHD was 1.42, 2.99 and 3.20 times of that in the base line group. The risk rate of getting hypertension was 1.63 and 1.92 times of that in the base line group. The systolic pressure and diastolic pressure were included in the effective factors to analyze again, which was still showed that the risk of getting illness of the upper diseases increased with the widen of pulse pressure. CONCLUSION: ① The prevalence rates of stroke, CHD and hypertension increase as the pulse pressure widens. �
