Analysis of gamma knife combined with whole brain metastases radiation therapy

摘要 目的评价脑转移瘤伽玛刀联合全脑放射治疗的价值.方法将62例脑转移瘤患者随机分为伽玛刀联合全脑放射治疗组(A组,30例)和全脑放射治疗组(B组,32例).A组先行伽玛刀1次照射治疗,周边剂量为18 Gy,中心最大剂量为32~36 Gy;再行全脑放射治疗,总剂量为30 Gy/10次.B组行全脑放射治疗,总剂量为30 Gy/10次,然后转移灶再追加9 Gy/3Fx放射治疗.分别对两组患者CT影像学变化及近期疗效进行统计学分析.结果A组头颅CT影像学显示肿瘤体积明显缩小29例(96.7%),临床症状改善明显26例(86.7%),均明显优于B组(P<0.05).结论伽玛刀联合全脑放射治疗脑转移瘤能提高脑转移瘤的局部控制率. Objective To assess the value of Gamma knife combined with whole brain radiation therapy for brain metastases. Methods 62 brain metastasis patients were divided into gamma knife with whole brain radiation therapy(WBRT) group(group A) ,and WBRT alone group(group B). The gamma knife treatment group had a single radiation of circumference dose of 18 Gy and a central maximum dose of 32~36 Gy, followed by 30Gy/10Fx whole brain radiation therapy. Group B underwent 30Gy/Fx whole brain radiation therapy and 9Gy/3Fx for metastatic loci. The CT imaging features and the short-term effects of the two groups were analyzed. Results The CT imaging changes and the short-term effect of the group A were better than those of group B(P〈0.05). Conclusions Gamma knife treatment combined with WBRT can improve the regional control rate of brain metastasis. ( Shanghai Med J, 2005,28:697-699)
