摘要
目的总结分析钢板内固定治疗股骨骨折失败的原因及对策。方法从几个方面回顾性分析近5年来钢板内固定治疗股骨骨折失败的病例资料。结果15例股骨骨折病例中内固定器材选用普通钢板5例,国产加压钢板10例,术后钢板断裂9例,钢板弯曲、螺钉拔出或断裂6例,发生骨折不愈合12例,再骨折3例;经采用加压钢板、髓内钉固定、植骨再次手术等措施均获痊愈。结论钢板内固定治疗股骨骨折失败原因较多,主要是医源性因素即技术性因素,钢板的选择及术后处理等方面也不容忽视。
[Objective] To summarize and analyze the causes and measures of invalidation of arrmour plate internal fixation in the treatment of femoral fractures. [Method] The clinical data of patients with femoral fractures, who underwent armour plate internal fixations and invalid outcome in recent 5 years, were retrospectively analyzed in several factors. [Result] Common armour plates were applied as internal fixation device in 5 of 15 cases with femoral fractures and domestic compression armour plates in 10. Postoperative plate rupture occurred in 9 cases. Armour plate curve and screw extraction or rupture occurred in 6 cases, not healing and refracture in 12, 3 cases respectively. Those patients all recovered after a series of measures in reoperation including compression armour plate and intrameduUary pin fixation, bone plant, etc. [Conclusion] There are considerable reasons for invalidation of armour plate internal fixation in the treatment of femoral fracture, of which main factor is medical or technical. In addition, several factors such as inappropriate armour plate and incorrect postoperative treatment should be considered.
