非穿透性小梁手术联合T—Flux植入治疗青少年型青光眼临床观察

摘要 目的 探讨非穿透性小梁手术联合T-Flux植入治疗青少年型青光眼的临床疗效。方法 对15例(19只眼)青少年型青光眼患者,行非穿透性小梁手术(nonpenetrating trabecular surgery,NPTS)联合丝裂霉素C(MMC)并植入T-Flux术,术后观察滤过泡、前房、眼内反应、眼压、视力、眼底及并发症。随访时间3周~30月,平均(8.9±7.7)月。结果 患者术前眼压21~50mmHg,平均(37.7±9.5)mmHg,术后1周平均眼压(9.6±3.4)mmHg(19只眼),术后1月平均眼压(15.5±3.3)mmHg(15只眼),术后6月平均眼压(14.0±3.0)mmHg(10只眼),术后10月平均眼压(15.0±2.9)mmHg(7只眼),术后1年平均眼压(14.8±3.8)mmHg(5只眼),术后2年平均眼压(18.0±2.0)mmHg(3只眼),术后2年半平均眼压(19.0±0)mmHg(1只眼),与术前相比差异有非常显著性(P<0.01)。复发需再手术1只眼,其余眼压均在21mmHg以下,成功率94.4%。视力与视乳头杯盘比(C/D)手术前后无明显变化。术后并发症主要有前房出血2只眼(10.52%)、I度浅前房2只眼(10.52%),未见滤过泡漏及睫状体脉络膜脱离,术中发生部分穿透5只眼(26.31%)。结论NPTS+MMC+T-Flux术是安全有效的治疗青少年型青光眼的理想方法。 Objectiye To evaluate the efficacy of nonpenetrating trabecular surgery With. T-Flux Implantation for treating juvenile glaucoma . Methods Fifteen eases (19 eyes) of juvenile glaucoma underwent nonpenetrating trabeeular surgery (NPTS) using Mitomyein C (MMC) with implantation of T-Flux. The postoperative filtering bleb,anterior ehamber,intraoeular reaetions,intraoeular pressure (IOP), visual acuity,Ocular Fundus and complication were recorded. The follow-up was 3 weeks to 30 months, meanly (8. 94±7.7) month. Results The preoperative IOP was 21-50mmHg,meanly (37.7±9.5)mmHg. The mean IOP was(9.6±3.4)mmHg in 19 eyes 1 week after operation, was (15.5 ± 3.3)mmHg in 15 eyes 1 month after operation, was (14.0 ± 3. 0)mmHg in 10 eyes 6 months after operation ,was (15.0 ± 2.9)mmHg in7 eyes 10 months after operation, was (14.8 ± 3.8)mmHg in 5 eyes 1 year after operation,was(18.0±2.0)mmHg in 3 eyes 2 years after operation, was(19±0)mmHg in 1 eye 30 months after operation, it is significant (P〈0.01)comparing with preoperative figures. Except 1 eye recrudesced and need another operation, the IOP of other eyes was below 21mmHg,the successful rate was 94.4%. There is no significant difference between the preoperative and postoperative visual acuity and optic papilla cup-toisc ratio(C/D). The main postoperative complications were hyphema (2 eyes account for 10. 52 %), shallow anterior chamber (2 eyes account for 10. 52 %), no filtering bleb leak and detachment of choroid were observed,partial penetrating occurred in 5 eyes during operation. Conclusion NPTS+ MMC+ T-Flux operative method is safe and effective. It is an ideal method for treating juvenile glaucoma.
