摘要
目的:探讨中枢神经系统自身免疫性疾病多发性硬化病因与发病机制,以38个实验包括1020例患者的资料作相关分析。资料来源:应用计算机检索PubMed和Ovid1980-01/2005-01与多发性硬化相关文章,检索词“etiology,multiplesclerosis”,“pathogensis,mul-tiplesclerosis”并限定语种为English。同时计算机检索CNKI数据库2000-01/2005-01与多发性硬化相关文章,检索词为“多发性硬化”,限定语种为中文。资料选择:对资料进行初审,选取有关多发性硬化病因、发病机制的相关文献,然后筛除明显不随机临床试验的研究,对剩下的文献查找全文,进一步判断是否为随机对照临床试验,纳入标准为随机对照临床试验,无论是否为单盲、双盲或非盲法。资料提炼:共收集到50篇关于多发性硬化病因发病机制的相关文献,38个实验符合纳入标准。资料综合:从地理因素、感染因素、遗传因素阐述多发性硬化的病因,从细胞因子、髓鞘脱失和轴索损害、自然杀伤细胞、细胞黏附分子、细胞凋亡与多发性硬化的关系阐述发病机制。结论:关于多发性硬化的病因、发病机制还无统一结论。细胞因子、髓鞘脱失、自然杀伤细胞等可能与多发性硬化的发病机制有关。
OBJECTIVE: To explore the etiological factor and pathogenesis of multiple sclerosis (MS) of autoimmunity disease in central nervous system, relative analysis is checked based on 38 trials including 1020 patients. DATA SOURCES: We searched the PubMed and Ovid for relative MS articles pubilished from January 1980 to January 2005 using the key searching terms "etiology,multiple sclerosis", "pathogenesis, multiple sclerosis" with the language limited to English. Meanwhile, the CNKI database was searched for relative MS articles published between January 2000 and January 2005 using the key term of multiple sclerosis with the language limited to Chinese. STUDY SELECTION: After the first selection, the relative literatures on the etiological factor and pathogenesis of MS, and then the articles containing obviously non-randomized clinical trial were excluded. The left literatures were searches for full texts to determine if it was the randomized controlled clinical trial. Inclusion criteria was randomized controlled clinical trial, whether single-blind, double-blind or non-blind method was used. DATA EXTRACTION: All the 50 articles on the etiology and pathogenesis of MS were collected and only 38 articles met the inclusion criteria. DATA SYNTHESIS: The etiological factor of MS was discussed from geographic factor, infective factor and genetic factor. The pathogenesis was explained based on the relation of cell factor, amyelination, nature killer cell, cellular adhesion molecule with apoptosis and MS. CONCLUSION: There is no uniform conclusion about the etiological factor and pathogenesis of MS. The cytokine, amyelination, nature killer cell etc.,maybe have a relation with the pathogenesis of MS.
