摘要
目的 评价吉西他滨联合适形放射治疗局部晚期胰腺癌的疗效.方法 前瞻性分析2003年4月至2005年6月收治的54例局部晚期胰腺癌患者的临床资料,其中25例采用吉西他滨联合适形放疗(试验组),29例采用吉西他滨单药化疗(对照组).结果 试验组和对照组的有效率分别为69.6%和39.3%,两组比较差异有统计学意义(P＜0.05);远处转移率分别为43.5%和53.6%,两组比较差异无统计学意义(P＞0.05);6个月生存率分别为78.3%和53.6%,两组比较差异无统计学意义(P＞0.05);12个月生存率分别为52.2%和25.0%,两组比较差异有统计学意义(P＜0.05);临床获益率分别为82.6%和60.7%,两组比较差异有统计学意义(P<0.05);严重不良反应发生率分别为43.4%和32.1%,两组比较差异有统计学意义(P<0.05).结论 吉西他滨联合适形放射治疗局部晚期胰腺癌,在近期有效率、12个月生存率方面优于单纯吉西他滨化疗,但在临床获益率、远处转移率、6个月生存率及严重不良事件方面,二者差异无统计学意义.
Objective To evaluate the effect of gemcitabine plus three dimensional conformal radiotherapy on patients with locally acvanced pancreatic cancer. Methods Fifty-four patients with locally advanced pancreatic cancer from April 2003 to June 2005 were enrolled, one of 25 patients were treated with gemcitabine plus three dimensional conformal radiotherapy (experiment group), the other 29 patients were treated only with gemcitabine (control group). Results The general remission rate was 69.6% in experiment group and 39.3% in control group, showing a significant statistical difference between two groups(P＜0.05)). There were no statistical difference between the two groups in metastases rate (43.5% vs 53.6%,P＞0.05), survival rate of six months (78.3%vs 53.6%, P＞0.05),clinical benefit response(82.6%vs 60.7%, P<0.05)and grave adverse events(43.4% vs 32.1%, P<0.05), but there were significant difference in survival rate of twelve months between two groups (53.6% vs 25.0%, P＜0.05). Conclusions Gemcitabine plus three dimensional conformal radiotherapy may be better than single chemotherapy of gemcitabine in general remission rate and survival rate of twelve months in patients with locally advanced pancreatic cancer, but there were no difference between two groups in clinical benefit response, metastases rate, survival rate of six months and grave adverse events.
