摘要
【目的】探讨18F-FDG SPECT显像不同重建方法对鼻咽癌影像的影响。【方法】鼻咽癌患者27例(初发12例,放疗后15例)进行18F-FDG符合线路显像,以预先分组最大期望值符合电路成像衰减校正(COSEM/AC)、非衰减校正(COSEM/NC)重建和滤波反投影(FBP)重建图像。比较三种方法的图像质量及病灶的探测情况。【结果】①FBP重建速度快,但质量差。COSEM重建速度慢,但影像质量高;经衰减校正后图像边缘结构清晰,反映组织的实际放射性分布。②COSEM/AC、COSEM/NC和FBP对鼻咽部恶性病灶的检出率分别为100%(22/22)、91.0%(20/22)和91.0%(20/22);三种方法对鼻咽外病灶检出率均一致。③COSEM/AC和COSEM/NC的L/B比值明显高于FBP(P<0.001)。COSEM/AC和COSEM/NC在头颈部病灶的L/B比值无显著性差异(鼻咽部:t=0.918,P=0.37;头颈部:t=1.793,P=0.079),但在胸腹部有显著性差异(t=3.955,P=0.001)。【结论】COSEN/AC重建在FDG SPECT显像中能提高影像质量和鼻咽部病灶的检出。
[Objective]To study the effects on ^18 F-FDG SPECT imaging of nasopharyngeal carcinoma(NPC) with different reconstruction techniques. [Methods]Tweenty-seven patients with NPC (12 primary cases and 15 cases after RT) were undergone ^18 F-FDG SPECT imaging. The images were reconstructed with COSEM/AC,COSEM/NC and FBP, the image qualities and the lesion detections were compared amongst these methods. [Results]①Reconstruction with FBP was fast and image quality was poor, while COSEM had slower reconstructed speed but the image quality was improved, and the edge structures of image with COSEM/AC were clear, which could exactly reflect the radioactivity distributions. ②the lesion detection rates in nasopharynx were 100% (22/22), 91.0% (20/22) and 91.0% (20/ 22) with COSEM/AC, COSEM/NC and FBP, respectively; and the same in carcinoma outside the nasopharynx. ③ The L/B ratios with COSEM/AC and COSEM/NC were higher than those with FBP( P 〈0. 001 ) ; there was no significant difference in the L/B ratio of the neck and head lesions between COSEM/AC and COSEM/NC nasopharynx: ( t =0. 918, P = 0.37) ;head and neck outside nasopharynx: ( t = 1. 793, P = 0. 079). But there was significant difference in the chest and abdomen (t= 3. 955, p= 0. 001 ). [Conclusion]The reconstruction with COSEN/AC in ^18 F- FDG SPECT imaging could improve image quality and increase lesion detection rate on nasopharynx in the patients with NPC.
