抗菌药使用与金黄色葡萄球菌耐药率变化相关分析 被引量:9

摘要 目的:了解抗菌药使用品种、数量与金黄色葡萄球菌耐药率之间的关系。方法:计算5年来我院每年抗菌药限定日剂量(DDD)累积数和平均每日每百张床位消耗的DDD数,以及同期金黄色葡萄球菌对10种抗菌药耐药率,并对抗菌药使用量与细菌耐药率进行相关分析。结果:5年来喹诺酮类、第2代头孢菌素、含β内酰胺酶抑制剂复合物、大环内酯类、糖肽类等抗茵药使用量逐年增加,而氨基糖苷类、第1、3代头孢菌素、青霉素类使用量变化不大;同期金黄色葡萄球菌对多种抗菌药耐药率有与之相对应的明显变化,对苯唑西林及其他多种抗菌药耐药率的上升与第2代头孢菌素、喹诺酮类、含β内酰胺酶抑制荆复合物等使用量呈正相关,而与青霉素类、第1、3代头孢菌素类和氨基糖苷类无关或负相关。结论:喹诺酮类、第2代头孢菌素和含β内酰胺酶抑制剂复合物使用量与金黄色葡萄球菌对苯唑西林耐药率的增加有较大的影响,在治疗G+菌感染时应根据病情轻重和药敏试验结果合理选用抗菌药。
