应用携带监测皮岛的腓骨移植重建股骨上段肿瘤术后骨缺损 被引量:2

Reconstruction of upside femur bone defects after bone tumor resection using a free vascularized fibu-lar transfer with a monitoring-flap

摘要 目的报道应用携带监测皮岛的腓骨移植修复股骨上段骨肿瘤术后骨缺损的临床效果。方法对于7例股骨上段肿瘤切除术后的骨缺损采用携带监测皮岛的腓骨进行移植。根据骨缺损长度,设计带监测皮岛(3cm×5cm)的腓骨,切除腓骨长度比骨缺损长度多4cm,上下各2cm插入股骨髓腔后用滑动鹅头钢板(DHS)桥接固定腓骨。结果全部病例获得随访12—24个月,无复发。术后功能恢复良好,腓骨和受区骨均呈骨性愈合。结论采用带监测皮岛的腓骨移植复合DHS固定是治疗股骨上段骨肿瘤术后骨缺损的有效方法。 Objective To explore the effect of treatment upside femur defects after bone tumor resection by the combinational fibular grafts with monitoring-flap by microvascular anastomosis. Methods Seven cases with bone tumor cases on upside femur after resection were reconstructed using a free vascularized fibular transfer with a monitoring-flap. According to the extent of bone defect, the extent of fibula with a monitoring-flap(3 cm×5 cm) was 4 cm longer than the extent of bone defect, The free vascularized fibula inserted in the futour marrow combined with DHS. Results All cases were followed up from 12 to 24 months. No recurrence was found. The postoperative function was well, Conclusion The therapy, that reconstruct the upside femur bone defects after bone tumor resection using a free vascularized fibular transfer with a monitorlng-flap combined with DHS,is reliable method.
