摘要
首次研究应用逆向法测定正常人前管内侧皮神经短潜伏期体感诱发电位(shortlatencysomatosensoryevokedpotentials),并研究其感觉动作电位(sensorynerveactionpotential)。方法:(1)SNAP:肱骨内上髁前外侧平均2.6cm处表面双极刺激,前臂掌面内侧平均10.3cm处记录SNAP。(2)SLSEP:上述刺激位置不变,用四道程记录锁骨上、颈,棘突、对侧顶区(P3/P1)相应的N9、N13、N20诱发电位.通过分别对N9、N13、N20的潜伏期、N9~N13峰间潜伏期与臂长的相关分析,认为峰间潜伏期及自身双侧对照的潜伏期差值和波幅比值为临床可靠指标,可望对胸廓出口综合征(TOS)早期诊断有指导意义。
This report describes an antidromic technique for measuring sensory nerve action potential(SNAP) of the medial antebrachial cutaneous nerve(MACN), and the technique for detecting the short latency somatosensory evoked potentials (SLSEP) of MACN in 15 (30 sides) normal subjects. Surface stimulation was done at the elbow 2~3. 5 (2. 6) cm anterolateral to the medial epicondyle. Recordings were made at the medial forearm 9. 5~11. 6(10'.3)cm distally for SNAP, at the Erb's point for N9, at the C7 cervical spinal process for N13 and at the contralateral scalp(P3/P4) for N20 (international 10~20 system). The respective latency, amplitude, conduction velocity were measured. The correlation analysis between the arm length and various evoked potentials showed N9~N13 conduction time and the difference between sides provided more reliable and sensitive clinical data. Such normal values of MACN may suggest an early objective test for diagnosing thoracic outlet syndrome (TOS).
