摘要
自１９９０年至１９９４年５月，用显微手术方法治疗４４例大脑前动脉系统动脉瘤，其中３９例为前交通动脉瘤，２例为额极动脉末端动脉瘤，位于大脑前动脉水平段、垂直段和胼周动脉起始部动脉瘤各１例。２例前交通动脉瘤还合并其它部位动脉瘤。６个梭形动脉瘤，余为囊性动脉瘤。６个梭形和３个囊性动脉瘤经电灼包裹，余中型囊性动脉瘤均由一次性手术夹闭瘤颈。术后２例死亡。全部病例随访。除１例植物状态、１例偏瘫、２例视野缺损外，余均恢复良好。我们认为病人的预后除与术前分级、手术时机有关外，还与动脉瘤囊突向不同部位和方向有关。本文还对不同部位该系统动脉瘤的手术入路及方法进行了分析探讨。
Abstract Since Jan. 1990, 44 cases of aneurysms arising from the anterior cerebral arterial system have been treated with microneurosurgical technique. They took 35.2 per cent of 125 cases of intracranial aneurysms during the same period.In this group, there were 39 aneurysms growing from the anterior communicating artery. 2 aneurysms arising form the distal of the frontopolar artery, there were 3 aneurysms located at the proximal anterior cerebral artery (A 1), the distal anterior cerebral artery (A2) and the origin of the callosomarginal artery (A3) respectively. In this group. there were 2 cases of multiple intracranial aneurysms. There were 6 fusiform aneurysms and the rest were saccular aneurysms. Except all of the fusiform aneurysms and 3 small saccular aneurysms were coagulated and wrapped, the rest of saccular aneurysms were clipped. After operations, 2 cases died, the mortality rate was 4.5 per cent. All patients were followed up. One patient is still in vegetative state. One is in hemiplegic paralysis. 2 patients still remained defects of visual fields. The rest of patients return to the normal life. We find that operative results relate not only to the grade of patients and the time of operations, but also to the directions of aneurysm fundus. The different operative approaches and operative techniques are also discussed.
