胆囊癌MVD计数及nm23H1,CD44V6表达的意义 被引量:3

摘要 目的:研究nm23H1,CD44V6基因蛋白和胆囊癌微血管密度与胆囊癌的发生、发展及胆囊癌恶性生物学行为的关系,并探讨三者之间有无相关性.方法:应用免疫组化SP法检测42例胆囊癌,20例胆囊腺瘤和20例慢性胆囊炎的nm23H1,CD44V6,CD34的表达情况,综合分析结果(CD34染色显示血管内皮细胞以检测上述标本的MVD).结果:42例原发胆囊癌中nm23H1和CD44V6的阳性表达率分别为60%和74%,均高于胆囊良性病变组(P<0.01).nm23H1在胆囊癌中的表达与胆囊癌分化程度、有无转移有关联性(P<0.05).CD44V6抗原阳性表达率中晚期癌显著高于早期癌,有转移组明显高于无转移组(P<0.05).胆囊癌中MVD值高于胆囊良性病变(P<0.05),MVD与胆囊癌的临床分期、转移有相关性(P<0.05).nm23H1,CD44V6表达与胆囊癌MVD之间无相关性.结论:nm23H1,CD44V6和MVD可能在胆囊癌的发生、发展中起重要作用.检测nm23H1和CD44V6的基因蛋白表达和MVD计数是反映胆囊癌生物学特性良好的指标,有助于区分胆囊良恶性病变,对判断胆囊癌的临床分期,浸润转移和预后具有一定的临床意义. AIM: To investigate microvessel density (MVD) and the expression of nm23H1 and CD44V6 in primary gallbladder carcinoma and to evaluate its relationship with the tumor oncogenesis and biological malignant behavior. METHODS: The specimens from 42 cases of primary gallbladder carcinoma, 20 cases of gallbladder adenoma, and 20 cases of chronic cholecystitis were analyzed by HE and PAS staining. SP immunohistochemical method was used to detect the expression of nm23H1, CD44V6 and CD34. RESULTS: The positive cell rate of nm23H1 in primary gallbladder carcinoma was 60%, while it was 25% and 10% respectively in adenocarcinoma and benign lesions of gallbladder. The positivity rate of nm23H1 was significantly higher in gallbladder carcinoma compared with that in benign lesions of gallbladder. There was close correlation between the expression of nm23H1 and differentiation degree and metastasis of gallbladder carcinoma (P<0.05). The positive cell rate of CD44V6 antigen in gallbladder carcinoma was significantly higher than that in gallbladder adenoma and chronic gallbladder inflammation (P<0.05), significantly higher at mid-late stage than at early stage in gallbladder carcinoma, and higher in patients with metastasis than patients without metastasis. The MVD in gallbladder carcinoma was significantly higher than that in gallbladder benign lesions (P<0.05). There were also correlation between MVD and clinical classification and metastasis of gallbladder carcinoma. There was no correlation between the expression of nm23H1, CD44V6 and MVD of gallbladder carcinoma. CONCLUSION: MVD, nm23H1 and CD44V6 expression are valuable indicators for the biological characteristics of primary gallbladder carcinoma and helpful in distinguishing malignant from benign tumors. They can be used as objective indicators to judge the clinical stage, invasion, metastasis and prognosis in gallbladder carcinoma.
