脑出血后TNF-α表达及地塞米松干预对其影响的临床研究 被引量:5

摘要 目的观察脑出血患者血液中肿瘤坏死因子-α(TNF-α)表达动态变化及干预治疗对其影响的临床意义。方法随机将74例脑出血患者分为常规治疗组、干预治疗组(加用地塞米松),健康体检者为对照组。所有病例于发病第1、3、7、14天清晨空腹抽取肘静脉血,测定TNF-α、单核细胞、白细胞总数、淋巴细胞和CSS评分变化。结果干预治疗组1d内血清TNF-α含量升高,第3天最高,第7天仍高于对照组(P<0.05),第14天与对照组基本一致。干预治疗组低于常规治疗组(P<0.05)。TNF-α含量与单核细胞密切相关。患者体温均值低于37℃,但是白细胞总数却高于正常上限值,单核细胞数增高最明显。结论前炎性细胞因子TNF-α与脑出血急性期的早期临床炎性病理生理过程密切相关,地塞米松可使血中TNF-α含量降低,炎性反应减轻。 Objective:To observe the serum dynamic change on concentration of tumor necrosis factor-α (TNF-α) of the patients after cerebral hemorrhage,and to analyze the influence of early inhibiting the inflammatory reaction by glucocorticoids on them.Methods:74 patients with intracerebral hemorrhage were divided into two groups:normal and dexamethasone treatment groups.30 healthy cases were adopted as control group.After episode,elbow venous blood were collected in the morning on the first day,third day,seventh day and fourteenth day respectively,and the variance of TNF-α,monoplast,leukocyte,lymphoidocyte,CSS score were observed.Results:The serum concentration of TNF-α raised on the first day after admission,and it was highest on the third day,on the seventh day it was still higher than the control group(P<0.05),it had no differences with the control group on the fourteenth day.That of the dexamethasone treatment group was lower than normal treatment group(P<0.05).The concentration of TNF-α were closely associated with monoplast.In the patients with intracerebral hemorrhage,body temperature was <37℃,but the count of the leukocyte was higher than the normal cases, in which monoplast raised significantly.Conclusion:Proinflammatory cytokine TNF-α is associated with the early inflammatory course of patients with acute intracerebral hemorrhage.Dexamethasone can reduce the serum concentration of TNF-α,inhibit the inflammatory response mediated by cytokines.
