摘要
目的通过正常健康人群尿硫氰酸盐(urinarythiocyanate,USCN)水平的研究,确定其正常参考值范围,并在丙烯腈(acrylonitrile,ANC)接触工人的职业健康监护中加以应用,以验证其有效性。方法正常对照组为同地区某发电厂的60名健康早班工人,接触组为某腈纶厂从事ACN作业的93名早班工人。分别于7时、11时和15时收集班前、班中和班末尿样。USCN分析方法采用吡啶-对苯二胺法,结果以尿肌酐(creatinine,Cr)水平进行校正。结果在不同采样时点吸烟组的USCN水平均比非吸烟组高出1倍(P<0.01)。USCN测定应采用班末尿,建议未经尿肌酐水平校正的USCN正常参考值:吸烟者为11.0mg/L,非吸烟者为6.0mg/L;建议经尿肌酐水平校正的USCN正常参考值:吸烟者为14.0mg/gCr,非吸烟者为5.5mg/gCr。各年龄组间USCN水平差异无显著性。以本研究的正常参考值判断ACN作业工人USCN水平,所有阳性率随着ACN接触水平的增加而增加。结论本研究目标人群USCN正常参考值(吡啶-对苯二胺法)在ACN作业工人职业健康监护中行之有效,能反映工人的ACN接触情况。
Objective After studying the reference value of urinary thiocyanate(USCN),to apply and verify its effectiveness in occupational surveillance of workers exposed to acrylonitrile(ACN). Methods Two groups were studied including a control group(60 male workers from a power plant)and the exposed group(93 male workers exposed to ACN).The urine samples were collected at 7∶00,11∶00 and 15∶00,and were assayed by modified Aldridges method(pyridine-p-phenylenediamine mixture).The results of USCN were adjusted by the level of urinary creatinine(Cr). Results The level of USCN in smokers was twice as much as in non-smokers(P<0.01).It was suggested that urine samples should be collected at the end of the shift.The reference values of USCN were 11.0 mg/L in smokers and 6.0 mg/L in non-smokers,respectively,if the results of USCN were not adjusted by Cr.The reference valued of USCN were (14.0 mg/g Cr) for smokers and 5.5 mg/g Cr for non-smokers,respectively,if the results of USCN were adjusted by Cr.No significant difference associated with the age in the level of USCN. Conclusions The reference value of USCN of the target healthy population is applied effectively in occupational surveillance of workers exposed to ACN,and it could indicate ACN exposure of the workers.
