摘要 目的研究缺血冷停跳、常温不停跳和室颤状态对于心肌保护的影响。方法24只SD大鼠建立改良式Langendorff离体鼠心灌注模型后,随机分为3组:冷停跳组、常温不停跳组、室颤组。分别给予3种不同处理:冷灌停跳、常温正常灌注下不停跳和电刺激诱发室颤。记录干预前,恢复后5、10、20、30、60min测定心功能指标(LVEDP、LVESP、LVDP、±dP/dt)、CK-MB含量、全程心电活动以及左室心内膜下心肌组织送电镜检查。结果常温不停跳组和室颤组的心肌舒张收缩功能、心肌酶CK-MB等指标明显优于冷停跳组,且差异具有统计学意义(P<0.01),心肌的电镜超微结构也有相应的差别。结论相对于冷停跳组,常温不停跳组和室颤组对心肌有更好的保护作用。 Objective The purpose of this study is to compare the myocardio injury during the processes of beating heart and ventricular fibrillation with normal supply and cardioplegic asystent. Methods 30 SD rat hearts on Langendorff mode were divided into three groups: hepothermic cardioplegia(HC) perfusion hearts, beating hearts with normal perfusion(BH), ventricular fibrillation hearts induced by electric stimulus with normal perfusion(VF). Observed indexes include: ±dp/dtmax, ECG, myocardial CK-MB in blood and myocardial ultra structure observation from electron microscope, etc. Results Beating heart and ventricular fibrillation groups showed improved myocardial contractility and relaxation(±dp/dtmax) as compared with cardioplegia group during ischemia and reperfusion period(P<0.05=. CK-MB leakage was reduced after reperfusion in all the three groups(P<0.01=, but there was no statistical significance among them(P>0.05). Myocardial electron microscope observation showed that myocardial ultra structure of group BH and VF was better than that of group HC. Conclusion This experiment suggests that both beating heart and ventricular fibrillation with normal perfusion process have distinctly better cardio-protective effects. There is not significantly different myocardio injury effect between beating heart and ventricular fibrillation if they are under normal perfusion.
