延肾胶囊对残肾肾小球系膜细胞凋亡及其细胞外基质代谢的影响

摘要 目的探讨“延肾胶囊”对残肾肾小球系膜细胞凋亡及其细胞外基质代谢的影响。方法将Wistar大鼠120只随机分为6组,A组为假手术对照组;B组为5/6肾切除;C、D、E组分别为5/6肾切除+低、中、高剂量延肾胶囊;F组为5/6肾切除+肾衰宁。术后A、B两组给予等容积生理盐水灌胃。定量喂养,自由饮水。于术后15、90天测定各指标,每组每时相点观察10只大鼠。结果术后B组血浆血管紧张素浓度、肾小球系膜细胞增殖指数和凋亡指数、肾组织TGF-β1、Col、Fas、FasL积分光密度等指标高于A组。C、D、E组血浆血管紧张素浓度、肾小球系膜细胞增殖指数、肾组织TGF-β1和Col积分光密度高于A组,但显著低于B组,随着延肾胶囊剂量增加,上述指标有逐渐降低的趋势;肾小球系膜细胞凋亡指数、Fas和FasL积分光密度显著高于A、B两组,随着延肾胶囊剂量增加,上述指标有逐渐升高的趋势。F组血浆血管紧张素浓度、肾小球系膜细胞增殖指数、肾组织TGF-β1和Col积分光密度低于B组,但高于D、E两组;肾组织Fas和FasL积分光密度显著高于A、B组,稍低于D、E两组;肾小球系膜细胞凋亡指数与B组无显著差异,显著低于C、D、E3组。结论“延肾胶囊”能显著抑制5/6肾切除大鼠残肾血管紧张素和TGF-β1的高表达,增加Fas和FasL的表达,抑制残肾肾小球系膜细胞过度增生,促进增生的系膜细胞凋亡,稳定残肾肾小球系膜细胞数量,减轻细胞外基质的堆积,呈剂量依赖性;“肾衰宁”能抑制系膜细胞增生,但不能促使增生的系膜细胞凋亡。 Objective To elucidate the effect of Yan-Shen Capsule on glomerular mesangial cell apoptosis and extracellular matrix metabolism of remnant kidney. Methods 120 adult Wistar rats were randomly divided into 6 groups with 20 rats in each group. Rats in group A were sham-operated; rats in groups B, C, D, E and F were 5/6 nephrectomized. After operation, Groups C, D, E were given lower 450mg/(kg·d), middle 900mg/(kg·d) and higher 1800 mg(kg·d) dosage of Yan-Shen capsule into stomach. Group F was given control drug, Shen-Shuai-Ning Capsule. Groups A and B were given same volume of saline. The rats were fed with standard big granule feed for 0.15g(g·d), and permitted to drink water freely. On day 15 and 90 after operation, plasma angiotensin Ⅱ (Ang Ⅱ) concentration, glomerular mesangial cell proliferation and apoptosis index, renal TGF-β_1, collagen Ⅳ (Col Ⅳ), Fas and Fas ligand (FasL) expression integral of optical density (IOD) were observed. There were 10 rats for each group and each time point. Results After operation, Ang II concentration, glomerular mesangial cell proliferation and apoptosis index, and IOD of TGF-β_1, Col Ⅳ, Fas and FasL were obviously higher in group B than group A (P<0.05). In groups C, D and E, Ang Ⅱ concentration, glomerular mesangial cell proliferation index, and IOD of TGF-β_1 and Col Ⅳ were higher than those of group A, but much lower than those of group B (P<0.05). The higher the dosage of Yan-Shen capsule was given, the lower the above markers would be. The mesangial cell apoptosis index and IOD of renal Fas and FasL in groups C, D and E were much higher than those in groups A and B (P<0.05), with a positive dose-effect relationship. Ang Ⅱ concentration, glomerular mesangial cell proliferation index, and IOD of TGF-β_1 and Col Ⅳ in group F were lower than those in group B (P<0.05), but higher than those of groups D and E.
