摘要
目的:探讨利凡诺及辅助药物用于疤痕子宫中期妊娠引产的安全性及有效性。方法:总结我中心2002年1月至2004年9月住院的75例疤痕子宫中期妊娠引产病例,并分为A组:单纯利凡诺羊膜腔内注射组31例;B组:利凡诺羊膜腔内注射后有规律宫缩时加用安定组25例;C组:利凡诺羊膜腔内注射同时给米索前列醇阴道后穹窿放置组19例。观察引产效果及其距前次剖宫产时间关系,各组引产方法、引产效果、引产时间、产时出血的比较及并发症发生情况。结果:75例引产成功率为96%,引产效果与距前次剖宫产间隔时间无明显关系。A、B、C三组一次性给药引产成功率分别为64.5%、80.0%、78.9%,B、C组稍高于A组(P>0.05),三组平均引产时间分别为54.21±3.07h、50.40±3.39h、38.67±4.29h,C组明显短于A组(P<0.01)。无一例引产出血、宫颈裂伤、先兆子宫破裂及子宫破裂等严重并发症发生。结论:利凡诺羊膜腔内注射、利凡诺加安定及利凡诺加米索前列醇用于疤痕子宫中期妊娠引产是安全有效的。且后两者能缩短引产时间,提高一次性给药引产成功率。
Objective:To assess the application and effectiveness of Rivanol and assists in terminating middle period gestation in the scar uterus. Methods: Seventy-five cases of induce labor for middle period of gestation in scars uterus in the hospital during Jan. 2002 to Sep. 2004 were retrospectively reviewed. The cases were divided into three groups. In group A, Simple Rivanol's amnion was injected inside the cavity (n = 31); in group B, there was law uterus contraction to add after Rivanol's amnion was injected (n =25) and in group C, Rivanol's amnion was injected inside the cavity and was laid up team 19 examples to the vault gallery behind the mellow wine vagina of Misprostol at the same time. The relation of previous caesarean section and induce labor effect, induce labor time and product bleeding and complications were observed. Results: There was 96% of successful induce labor in 75 patients that was not relation with privous cesarean section. There were 64.5%, 80.0% and 78.9% of successful induce labor in Groups A, B, C, respectively, with once administration of medicine. The rates of groups B and C showed slightly higher than that in group A (P>0.05). The average induce albour time in the three groups were 54.21± 3.07 hours, 50.40 ±3.39 hours and 38.67 ±4.29 hours respectively, and group C sowed a marked shorter than group A (P<0.01=. There were no induce albour bleeding and other serious complications such as wound and omen uterus burst and the uterus bursts. Conclusion: Injection of Rivanol's amnion inside the cavity, Rivanol oplus Valium, and Rivanol plus the mellow wine in Misprostol may be useful and safe for the scar uterus middle period gestation induce albour. Both can shorten induce labor time and raise once-medicine success rate.
