摘要
目的:评价以自体筋膜为固定材料治疗实验性腰椎滑脱模型的可靠性。方法:以7具新鲜人体腰椎运动节段标本为对象,按正常、滑脱和筋膜固定组顺序,在MTS测试系统上进行载荷下屈伸试验。获得各组首次载荷循环及每隔250次,共5000次循环的运动范围和初始刚度值。结果:滑脱组活动范围大于正常组和固定组,有显著性差异(P<0.01);首次及第5000次循环载荷,正常组与固定组无显著性差异(P>0.05)。首次循环载荷下,滑脱组屈伸初始刚度均小于正常组和固定组,有显著性差异(P<0.01),正常组与固定组无显著性差异(P>0.05);第5000次循环载荷,正常组屈伸初始刚度与固定组均无显著性差异(P>0.05)。结论:以自身筋膜为固定材料来治疗实验性腰椎滑脱模型,可获得术后即时稳定并能在循环载荷下保持其固定效果。
Objective: To evaluate the biomechanical characteristics of the posterior stabilization of the lumbar spondylolisthesis fixed with auto-graft fascia under cycle loading. Methods: 7 fresh human lumbar spine (L4~L5) were divided into three groups in order: normal, spondylolisthesis and spondylolisthesis fixed with auto-graft fascia. All groups were underwent the experiment of loading flexion-extension with the 858.02 mini bionix test system. The initial stiffness and the move range were measured at the initial loading cycle and 5000 cycles for 250 cycle intervals. The results were statistically analyzed. Results: Move range of spondylolisthesis group was significantly greater than that of normal group and auto-graft fascia group (P<0.01). During the first and 5000th loading cycle, the move range of normal group also greater than auto-graft fascia group, but no significance (P>0.05). During the first loading cycle, the initial stiffness of spondylolisthesis group on flexion-extension was smaller than that of normal group and auto-graft fascia group (P<0.01), and that of normal group smaller than auto-graft fascia group, but no significance (P>0.05). For 5000 loading cycle, the initial stiffness of normal group was smaller than auto-graft fascia group on flexion, however, that of normal group was greater than auto-graft fascia group on extension, but all no significance (P>0.05). Conclusions: For fixing the lumbar spondylolisthesis and maintaining its stability effectively during cycle loading, the method of the lumbar spondylolisthesis fixed with auto-graft fascia can provide biomechanical stability for clinical application.
