摘要 目的研究和评价HypnoPTT睡眠呼吸监测仪对阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的诊断价值。方法选择147例打鼾患者,对其行整夜多导睡眠监测(PSG),以PSG的呼吸暂停低通气指数(AHI)为诊断OSAHS的金标准,同时对同一患者作HypnoPTT监测获得其监测结果PTTAHI。绘制受试者工作特征曲线(ROC曲线),获得PTTAHI的最佳诊断点,对PTTAHI作分层检测,得出各层的阳性似然比。结果PTTAHI与金标准PSGAHI的结果有较好的直线相关关系。以PSGAHI≥5作为目前国内诊断OSAHS的金标准,绘制ROC曲线,得出的PTTAHI对OSAHS的最佳诊断值为PTTAHI≥8。对该诊断点作卡方检验,其灵敏度为97.5%,特异度为90.5%。结论PTT监测仪对OSAHS的诊断准确性较高,临床应用价值较大。 Objective To evaluate a new portable sleep respiratory recording device (Hypno PTT) to diagnose obstructive sleep apnoea/hypopnoea syndrome (OSAHS).Methods 147 suspected OSAHS patients were enrolled and conducted nocturnal PSG and HypnoPTT.The apnoea pulse hypopnoea index (PTT-AHI,events/h) recorded by Hypno PTT was compared with that recorded by PSG (PSG-AHI).The diagnostic capabilities of Hypno PTT were compare with full polysomnography (PSG).Results There were significant correlation between the indices recorded by PTT-AHI and PSG-AHI (r=0.973;P=0.000).With the PSG-AHI≥5 as the diagnostic standard of OSAHS,a received operating characteristic (ROC) curve was drawn.Based on ROC curve a cut-off point of 8 was identified as the best point for PTT to diagnose OSAHS with a sensitivity of 97.5% and a specificity of 90.5%.Conclusion Hypno PTT is an inexpensive and feasible device to diagnosis OSAHS with highly sensitivity and specificity.
