摘要
目的:检测聚集素(clusterin)和B细胞淋巴瘤/白血病-2基因(Bcl-2)在血管瘤和血管畸形中的表达,探讨他们在血管瘤自发消退过程中的作用。方法:采用免疫组化SP法检测30例增殖期、22例消退期血管瘤、27例血管畸形及5例正常皮肤组织中clusterin和Bcl-2的表达;用TUNEL法测定内皮细胞凋亡率。结果:clusterin在血管瘤消退期的表达明显高于增殖期、血管畸形和正常皮肤(P<0.01);Bcl-2在血管畸形中表达高于正常,血管瘤消退期低于正常(P<0.01);内皮细胞凋亡率在血管瘤消退期明显高于增殖期、血管畸形和正常皮肤(P<0.01)。在血管瘤中,内皮细胞凋亡率与clusterin的阳性率呈正相关而与Bcl-2呈负相关。结论:clusterin在血管瘤中发挥促凋亡作用,血管瘤的自然消退与其有关;Bcl-2发挥抗凋亡作用。
Objective: To determine the expression of cell-aggregating factor (clusterin) and Bcl-2 in hemangioma and vascular malformations. Methods: The specimens were taken from 30 proliferative hemangiomas, 22 involuting hemangiomas,27 vascular malformations and 5 samples of normal skin. The expression of clusterin and Bcl-2 in endothelial cells were investigated by immunohistochemical SP method; apoptotic endothelial cells were identified by TUNEL method. Results: The expression of clusterin was significantly higher in proliferative hemangiomas than that in involuting hemangiomas, vascular malformations and normal skin (P<0.01). Bcl-2 was higher in vascular malformations than in normal skin and lower in involuting hemangiomas than in normal skin. The differences were statistically significant (P<0.01). The apoptosis rate of endothelial cells was much higher in proliferative hemangiomas than other groups. In hemangioma, the apoptotic rate was positively associated with clusterin(r=0.821, P<0.01)while negatively with Bcl-2(r=-0.406, P<0.01). Conclusion: Clusterin might play a promotive role of apoptosis during the development of hemangioma. There is an intimate relationship between clusterin and spontaneous regression of hemangioma. Bcl-2 plays an anti-apoptotic role.
