摘要
目的研究急性一氧化碳中毒(ACOP)患者血小板膜糖蛋白的变化以指导临床抗血小板活化药物治疗。方法分组包括急性一氧化碳中毒(ACOP)组,急性一氧化碳中毒高压氧(ACOP+HBO)组,正常对照组。HBO组由ACOP组中接受HBO综合治疗达10d以上的患者组成。ACOP组在来院后即刻采血,ACOP+HBO组在治疗第3天、第7天、第10天3次晨8时空腹采血,对照组晨8时分别空腹采血2ml,迅速送检,用流式细胞仪检测血小板膜糖蛋白CD63,PAC-Ⅰ和CD62p的表达。结果ACOP组来院后即刻血小板膜糖蛋白CD63,PACⅠ和CD62p的表达水平均高于正常对照组(P<0.01)。ACOP+HBO组CD63和CD62p表达减少,PAC-Ⅰ有下降趋势。结论ACOP可显著增加CD63,PAC-Ⅰ和CD62p在血小板膜上的表达,且CD63和PAC-Ⅰ在一段间內将维持较高水平崾続COP后抗血小板活化治疗在临床症状缓解后应继续进行一段时间。
Objective To study the effect of HBO treatment on platelet membrane glycoprotein in patients with acute carbon monoxide poisoning in order to instruct the anti-platelet therapy of ACOP . Methods Subjects were divided into ACOP group , HBO group and normal control group.15 patients in the ACOP group who received 10 days therapy of the HBO formed the HBO group.Blood was drawn insistently from patients in the ACOP group when they arrived at hospital.Drawn from patients in the HBO group at 8 AM after an overnight fast on the 3rd, the 7th, the 10th day after acute carbon monoxide poisoning;that was drawn from the normal control group at 8 AM after an overnight fast. The positive percentage of CD63, PAC-Ⅰ and CD62p were measured by the flow cytometry (FCM). Analysis of t test, variance(ANOVA) and Q test were applied to analyse the expressions of platelet membrane glycoprotein CD63, PAC-Ⅰ and CD62p. Results The expressions of platelet membrane glycoprotein CD63, PAC-Ⅰ and CD62p of the ACOP group were much higher than these of the normal control group(P<0.01). After the treatment of HBO,the expressions of CD63 and CD62p decreased, the expression of PAC-Ⅰ showed a downward trend. Conclusions The expression of CD63, PAC-Ⅰ and CD62p on the platelet membrane significantly increased after acute carbon monoxide poisoning. According to this study, the expressions of PAC-Ⅰ and CD63 maintained a high level in the acute period. It indicates that the inhibition of platelet activation is needed and should be applied for a period of time after the remission of clinical symptoms.
