摘要 目的 探讨经皮椎间盘内电热治疗(IDET)对椎间盘源性腰腿痛的临床疗效。方法 对4 0例4 1个椎间盘行CT和“C”臂X光透视引导下IDET治疗的临床疗效予以回顾性分析。采用WHO疼痛缓解标准[6 ] ,将疼痛缓解程度分为完全缓解(CR) ,部分缓解(PR) ,轻微缓解(MR)和无缓解(NR)。结果 39个椎间盘穿刺成功并做IDET治疗。椎间盘电热治疗后即刻有轻微缓解31例(31 / 38) ,无缓解7例(7 /38)。术后随访在1~7d内的有效率为5 5 %,7d~1个月内的有效率为34%,术后1~3个月随访有效率为76 %,术后3~6月随访总体有效率达87%。2例L5~S1椎间盘突出因穿刺失败而未行治疗。结论 IDET适于治疗椎间盘退行性变破裂引起的慢性腰腿痛。疗效和疼痛缓解随着术后时间的延长而逐渐明显。 Objective To discuss the effect of IDET treatment for low back pain. Methods Retrospectively analysed the treatment results of 40 cases 41 sections of interdiscal protrusion under CT and C arm X-ray guidance. Two cases of L5-S1 intradiscal protrusion were failed and 38 cases were successful with IDET. Results We adopted WHO criteria of pain relief and palliative care. Fifty-five percent cases were CR or PR between 1 day and 1 week postoperatively, and 34% cases had CR or PR between 1 week and 1 month postoperatively, also 76% cases between 1 and 3 months postoperatively, and 87% cases within 3 to 6 months postoperatively. Conclusions IDET is suitable for chronic low back pain caused by intrediscal rupture. Postoperative healing includes 4 stages: 1 to 7 days is the release stage, 7 days to 1 month is the discal edema stage, 1 to 3 months is the discal rehabilitation stage and 3 to 6 months is the discal stable stage.
