摘要
目的 研究放射性药物唾液腺显像定量分析对干燥综合征的诊断价值。方法 静脉注射99 TcmO4- 370 MBq,前位采集唾液腺动态图像,连续50 min。在40 min时,给受检者口含Vit C 0.5 g刺激唾液腺的分泌。根据唾液腺动态图像,建立腮腺和颌下腺的时间放射性曲线,并计算有关功能参数。结果 干燥综合征患者腮腺和颌下腺的摄取比值(UR)分别为2.8±1.2和1.6±0.7,正常对照组分别为5.1±1.4和3.4±0.6,两组比较有显著性差异(P<0.01);干燥综合征患者腮腺和颌下腺的Tmin分别为3.9±2.5和5.1±2.3 明显高于正常对照组的1.8±1.1 和2.4±1.4(P<0.01);干燥综合征患者腮腺和颌下腺的Tmax与正常对照组无显著性差异(P>0.05)。结论 测定UR和Tmin是判断干燥综合征患者腮腺及颌下腺功能十分灵敏的指标;而测定干燥综合征患者腮腺及颌下腺的Tmax临床价值不大。
Objective To study the diagnostic value of quantitative analysis of radiopharmaceutical imaging of salivary gland on Sjgren's syndrome. Methods After intravenous injection of 370 MBq 99 Tc m sodium pertechnetate, anterior dynamic salivary gland images were obtained for 50 min. Forty minutes after injection, 0.5 g Vit C were administered intraorally as a stimulus. A time-activity cure of each salivary gland was drawn and functional parameters were calculated with dynamic salivary gland images. Results The uptake rate (UR) of the parotid and submandibular glands in patients with Sjgren's syndrome was 2.8±1.2 and 1.6±0.7, that of normal controls was 5.1±1.4 and 3.4±0.6, respectively (P< 0.01 ). The T_ min of the parotid (3.9±2.5) and submandibular glands (5.1±2.3) significantly increased in Sjgren's syndrome compared with normal controls (1.8±1.1 and 2.4±1.4,P<0.01). The T_ max of the parotid and submandibular glands in patients with Sjgren's syndrome was not different compared with normal controls. Conclusion Determining UR and T_ min are very sensitive mark to estimate function of the parotid and submandibular glands in patients with Sjgren's syndrome; There is not clinical value on analysing T_ max of salivary gland in patients with Sjgren's syndrome.
