摘要
用99mTc-甲酯异两基异腈(CPI)对23例行心肌灌注门电路触发断层显像研究,能清楚地显示心肌边缘、厚度和放射性分布.经与静息断层比较,门控断层可显著提高缺血心肌节段的诊断灵敏度,检出率提高17.7%(P<0.01).经计算机处理可获得左心室收缩分数,与核素平衡法门电路心血池显像(r=0.7054,P<0.01)和超声心动图法(r=0.6812,P<0.05)测得的左心室射血分数呈有意义的中度相关,可作为反映左心功能的指标.还可测定舒张状态心肌容积,反映心脏收缩过程中心肌形态和放射性分布变化,具有反映室壁运动的潜力.
Abstract In this paper, we present the results of the gated SPECT on the myocardium using 99mTc-CPI for 23 cases. The myocardial fringe, thickness and radioactive distribution were shown clearly. The results showed that the diagnostic ability of gated SPECT in regional myocardial ischemia was superior to that of the SPECT (the positive rate enhanced up to 17.7%). The values of the diagnostic sensitivity for ≥45% stenosis of LAD, LCX, RCA and the total were 100%, 40%, 100% and 85.7% respectively by the gated SPECT and were 90%, 0, 83.3% and 66.7% respectively by the SPECT. We could obtain the left ventricle systolic fraction (LVSF) through the data of the gated SPECT by computer. The LVSF had a close correlation with the LVEF (left ventricle ejection fraction) obtained by the multi-gated blood pool imaging (r= 0.7045, P< 0.01) and the ECG (r= 0.6812, P<0.05). The average myocardial diastolic volume was 275.3 ± 63.4ml in 6 normals and 264.5 ± 49.1ml in 16 CADs.
