摘要
目的:屈肌腱愈合的过程是一个复杂的过程,这其间有大量的生长因子及其他物质参与,并且这些物质在发挥本身功效时决不是孤立的,而是相互影响、相互制约的构成一个体系,但是就这个多因素体系的框架而言,如何选择合适的给药方式及载体,从而更好的促进屈肌腱的愈合和功能康复,在近年来的基础与临床实验研究中已经成为热点。资料来源:应用计算机检索PubMed1997-01/2004-09文献,检索词tendonhealing,growthfactors,限定文章语言种类为English;同时检索http://www.wanfangdata.com.cn2001-01/2004-09文献,检索词“肌腱愈合,生长因子”,限定语言种类为中文。资料选择:对资料进行初审。纳入条件:①实验研究。②不限定对照及盲法。排除条件:同一领域重复研究和综述文献。资料提炼:从40篇关于生长因子在体内或体外肌腱损伤愈合模型中作用的研究文献中选出符合纳入标准的27篇。资料综合:内源性和外源性生长因子在肌腱愈合中的调节作用有:①给予损伤肌腱的大鼠模型修复部位注射血管内皮生长因子与对照组相比,能明显提高肌腱早期张力强度且能够促进转化生长因子β的表达。②针对碱性成纤维细胞生长因子mRNA在兔跟腱的表达检测,结果显示正常对照组(未受损)仅出现低水平表达,而实验组伤后第1天即出现明显表达,第7天达最高峰,维持2个月后下降到一个较低水平;另有研究发现碱性成纤维细胞生长因子在屈肌腱及腱鞘中的水平在术后1~58d(8周)中逐渐升高。③有研究表明转化生长因子β1的mRNA水平在受损的肌腱和腱鞘中都有增高,同时转化生长因子β受体的水平在损伤后也有上调,且与转化生长因子β1有着相同的分布。结论:屈肌腱愈合是一个复杂的过程,多种生长因子在肌腱损伤修复中起重要作用。今后的研究重点将更突出在寻找外源性生长因子的最佳给药途径以及
OBJECTIVE:Flexor tendon wound healing is a complicated process,accompanied by participation of multiple growth factors and other substances,and these substan ces form a system in which they play their roles not in an independent but inter acting and inter- restricting manner.However,as for the frame of such a system containing multiple factors,how to select a suitable medication method and carri er has become a hot topic in recent basic and clinical experiments so as to acce lerate flexor tendon wound healing and improve functional rehabilitation. DATA SOURCES:A computer- based search for relevant articles between January 1997 and September 2004 was performed in PubMed by using the key words of “ ten don healing,growth factors” ,and the language was limited to English.Meanwhile, another computer- based search for such articles between January 2001 and Septe mber 2004 was performed in http://www.wanfangdata.com.cn by using the same key w ords as above in Chinese. STUDY SELECTION:All articles were selected firstly based on the inclusion cri teria:① experimental studies, ② not limiting control and blind method,and ba sed on the exclusion criteria:repeated studies and reviews in the same field. DATA EXTRACTION:Twenty- seven articles accordant with inclusion criteria wer e collected from 40 articles about the research on the role of growth factors in the models of tendon wound healing in vitro and in vivo. DATA SYNTHESIS:Endogenous and exogenous growth factors have regulatory roles in tendon wound healing including:① As compared with the controls,the injection of vascular endothelial growth factors into the wounded tendon of rat models pr omoted the early tensile strength and increase the expression of transforming gr owth factor β (TGF- β )mote obviously.② In the detection of basic fibroblast growth factor(bFGF) mRNA expression in rat tendons,low expression of bFGF mRNA was found in the normal control(unwounded) rats,but expression of bFGF mRNA was obvious in the experimental rats from the first day,pea
