两种激光疗法对高度近视视力恢复及角膜上皮修复的比较

摘要 目的:准分子激光上皮下角膜磨镶术是一种新的屈光手术方法,观察准分子激光上皮下角膜磨镶术和准分子激光原位角膜磨镶术在高度近视治疗视力恢复及角膜上皮修复情况。方法:2003-06/2004-09哈尔滨医科大学第一附属医院眼科视光中心分别对准分子激光原位角膜磨镶术患者140例(272眼)和准分子激光上皮下角膜磨镶术患者120例(229眼)进行6~12个月的术后观察。结果:准分子激光上皮下角膜磨镶术患者术后36例(30%)出现疼痛、流泪、异物感等刺激症状,重于准分子激光原位角膜磨镶术,两者差异有显著性意义(P<0.01)。准分子激光原位角膜磨镶术术后245眼(90.1%)裸眼视力在4.7以上,224眼(82.4%)在4.9以上。266眼(97.8%)矫正视力在4.7以上,249眼(91.5%)矫正视力达5.0。267眼(98.2%)达到术前预测视力,其中24眼(8.8%)超出了术前最好矫正视力。准分子激光上皮下角膜磨镶术术后197眼(86.0%)裸眼视力在4.7以上,170眼(74.2%)在4.9以上。220眼(96.1%)矫正视力在4.7以上,211眼(92.1%)矫正视力达5.0。225眼(98.3%)达到术前预测视力,其中36眼(15.7%)超出了术前最好矫正视力。准分子激光上皮下角膜磨镶术患者134眼(58.4%)角膜上皮术后3d完全修复,4眼(1.7%)角膜上皮术后7d完全修复。准分子激光上皮下角膜磨镶术术后1个月10眼(4.4%)出现角膜上皮下混浊(Haze)I级,8眼(3.5%)出现HazeⅡ级,其余患者无Haze出现。结论:准分子激光上皮下角膜磨镶术避免了准分子激光角膜切削术与准分子激光原位角膜磨镶术的缺点,是一种安全、有效、具有发展潜力的近视眼治疗方法。 AIM:Laser epithelial keratomileusis(LASEK) is a new keratorefractive procedur e for the correction of myopia and myopic astigmatism.We observed the recovery o f high myopia and repair of corneal epithelium by laser in situ keratomileusis(L ASIK) and laser epithelial keratomileusis(LASEK). METHODS:Totally 140 patients(272 eyes) treated with LASIK and 120 patients(22 9 eyes) treated with LASEK in the Center for Optometry in the Department of Opht halmology,First Affiliated Hospital of Harbin Medical University between June 20 03 and September 2004 were followed up for 6 to 12 months postoperatively. RESULTS:Thirty- six LASEK patients(30% ) were found such irritation as pain ,tearing,and foreign body sensation.The irritation was significantly severer in the LASEK patients than in the LASIK patients(P< 0.01).During 6 to 12- month fo llow- up after LASIK,the naked vision over 4.7 was in 245 eyes(90.1% ) and ov er 4.9 in 224 eyes(82.4% );the corrected visual acuity over 4.7 was in 266 eyes (97.8% ) and up to 5.0 in 249 eyes(91.5% );267 eyes(98.2% ) were found up to preoperative predicting visual acuity and 24 eyes(8.8% ) better than preoperati ve best spectacle- corrected visual acuity(BSCVA).During 6 to 12 months of foll ow- up after LASEK,the naked vision over 4.7 was in 197 eyes(86.0% ) and over 4.9 in 170 eyes(74.2% );the corrected visual acuity over 4.7 was in 220 eyes(96 .1% ) and up to 5.0 in 211 eyes(92.1% );225 eyes(98.3% ) were found up to pre operative predicting visual acuity and 36 eyes(15.7% ) better than preoperative BSCVA.Treated with LASEK,the corneal epithelium of 134 eyes(58.4% ) cured afte r 3 days and 4 eyes(1.7% ) cured after 7 days.Grade Ⅰ Haze was observed in 10 eyes(4.4% ) and Grade Ⅱ Haze in 8 eyes(3.5% ) at 1 month after LASEK,and no Haze was found in the rest eyes. CONCLUSION:LASEK is a potential method to treat eyes with high myopia safely and effectively,which may combine advantages and eliminate disadvantages of phot orefractive keratectomy and LASIK.
