The leading cause of corneal graft failure'is T cell-mediated immune rejection. Costimula-tion has been shown to be necessary for full activation of naive T cell. The interaction of CD40-CD154 plays important roles in corneal allograft rejection, and blockade of which has been demonstrated to result in prevention of allograft rejection. In this article, the molecule characters and immune function of CD40-CD154 costimulatory signal were reviewed, and the effects and mechanisms of blockade of this pathway on survival of allogeneic corneal transplantation were mainly discussed.
