摘要
目的:评价保留二尖瓣后瓣及瓣下结构对瓣膜置换者术后左心功能的影响。方法:对80例二尖瓣置换患者不同术式术后心功能恢复状况进行研究,40例保留二尖瓣后瓣及瓣下结构(A组),另40例行全瓣切除瓣膜置换术(B组),术前、术后10天和术后3个月分别应用彩超监测A、B两组左心功能指标。结果:A组左室内径、左室功能较B组改善显著(P<0.05)。结论:瓣膜置换术中保留二尖瓣后瓣及瓣下结构有利于改善瓣膜置换术后左心功能。
Objective: To evaluate the effect of preservation of the posterior and subvalvular apparatus during valve replacement on left ventricular functions. Methods: The left ventricular recovery of 80 patients who had undergone mitral valve replacement with or without preservation of the posterior and subvalvular apparatus was investigated .The 80 cases divided into two groups. In 40 cases the posterior and subvalvular apparatus of mitral valve (group A) were preserved while the rest of the cases underwen conventional mitral valve replacement (group B). Parameters of left ventricular function of echocardiographic measurements were compared between group A and B at preoperative, early postoperative period(10 days after surgery ) and late postoperative period (three months after surgery). Results: Group A showed the better improvement of LVESD, LVEDD,LVFS and LVEF than group B (P<0.05). Conclusion: The preservation of posterior and subvalvular apparatus improves the left ventricular functions after valve replacement.
