摘要
目的观察创伤后炎症反应综合征(SIRS)病人中性粒细胞(PMN)的热休克蛋白(HSP)70表达、凋亡及功能状态的变化。方法22例创伤后SIRS病人,分别于SIRS前、后不同阶段测定PMN的HSP70表达、呼吸爆发功能、24h凋亡率及血浆细胞因子(IL-6、IL-8)水平,并与14例健康者对照比较。结果①创伤后PMN的HSP70合成与mRNA表达即开始增加,创伤后各时段HSP70合成与mRNA表达量均高于健康人水平;SIRS发生后表达水平明显高于SIRS发生前水平;②创伤后SIRS病人PMN24h凋亡率低于正常组;③创伤后PMN呼吸爆发功能增强,各时段O2-产生能力均高于健康人水平;④血浆IL-6和IL-8水平在创伤后0~3d无明显变化,至SIRS发生后开始逐渐升高并维持较高的水平。结论创伤后伴随着以炎症细胞因子升高为特征的SIRS发生,PMN发生了热应激反应及功能的变化,提示PMN的HSP70表达可能与其功能变化存在着一定的关系。
Objective To investigate the expression of HSP70 and HSP70mRNA, oxidative activity and extent of apoptosis of PMNs in the patients with SIRS suffering from trauma. Methods 22 patients with SIRS suffering from trauma were included. We collected blood samples during the course of recovery to investigate the expression of HSP70 and HSP70mRNA, oxidative activity and extent of apoptosis in PMNs. In parallel, the levels of cytokines (IL-6, IL-8) in blood plasma were also measured. We chose 14 healthy volunteers as control. Results ①The expression of HSP70 and HSP70mRNA and oxidative activity in PMNs patients were significantly increased. After SIRS occurring, the levels of HSP70 expression were higher than those before SIRS.② Apoptosis of these PMNs was markedly inhibited after trauma. ③The levels of cytokines (IL-6, IL-8) in blood plasma were increased with the development of SIRS. Conclusions The development of SIRS after trauma is characteristic of elevation of some inflammatory cytokines, and heat stress response and functional changes will occur in PMNs, which indicates HSP70 expression may relate with the changes of neutrophil functions.
