脑干听觉诱发电位对脑损伤听力障碍的客观评估 被引量:4

摘要 目的:分析脑干听觉电位判断听力损伤程度的客观性、准确性。方法:对1997-03/2004-07青海省人民医院神经外科门诊收治的脑损伤后听力下降患者106例166只耳,应用脑干听觉诱发电位检测,并分析其正常潜伏期和峰间期数据特征。结果:按意向处理分析,106例166只耳全部进入结果分析。71只耳为Ⅴ波反应阈异常,Ⅰ波全部缺失;44只耳为Ⅴ波反应阈异常而Ⅰ~Ⅴ波潜伏期正常;32只耳为表现为Ⅰ,Ⅲ,Ⅴ波引不出波形;15只耳为Ⅰ~Ⅲ波峰间延长,Ⅳ~Ⅴ波潜伏期延长。听力损伤耳的各波潜伏期(Ⅰ,Ⅲ,Ⅴ)和峰间期(Ⅰ~Ⅲ,Ⅰ~Ⅴ,Ⅲ~Ⅴ)与脑干听觉诱发电位正常潜伏期和峰间期相比较,差异有显著性意义(t=2.541~2.332,P<0.01)。结论:通过对患者脑干听觉诱发电位的检测能够反映从耳蜗至中脑听觉传导径路的相应神经结构的生物电变化,可根据此判断评价患者客观听力和相关部位的损伤程度。 AIM:To analyze the objectivity and veracity of brain stem auditory evoked potential in judging the severity of hearing impairment.METHODS:A total of 106 patients (166 ears) with hearing impairment following brain injury were selected from the Outpatients of Neurosurgery, Qinghai People' s Hospital from March 1997 to July 2004.Brain stem auditory evoked potential was detected, and the data in latent period and interspike interval were analyzed.RESULTS: All 106 patients (166 ears) were involved in the result analysis. Among them, 71 ears showed abnormal auditory threshold in Ⅴ wave, and missedⅠ wave; 44 ears showed abnormal auditory threshold in Ⅴ wave, but normal latent periods of Ⅰ - Ⅴ wave; 32 ears showed no waveforms in Ⅰ , Ⅲ , and Ⅴ wave; 15 ears showed prolonged interspike interval in Ⅰ - Ⅲ wave and latent period in Ⅳ - Ⅴ wave. There was significant difference between the latent period(Ⅰ ,Ⅲ ,Ⅴ wave) and interspike interval(Ⅰ - Ⅲ ,Ⅰ - Ⅴ ,Ⅲ - Ⅴ wave) in ears with hearing impairment and normal latent period and interspike interval of brain stem auditory evoked potential (t=2.541~ 2.332, PCONCLUSION: Bioelectricity changes in neural structure from cochlea to midbrain can be showed by the brain stem auditory evoked potential, which can be used to judge the objective audile of patients and the severity of impairment in the corresponding parts.
