摘要 目的:了解中老年人骨代谢生化指标随年龄变化的特点,分析骨代谢生化指标在骨质疏松症诊断评估中的价值。方法:选择2002-10/2004-03华中科技大学同济医学院附属同济医院综合科门诊和住院的中老年体检患者410例为研究对象。测定410例中老年人腰椎正位骨密度,同时检测所有患者空腹血清骨钙素、血清碱性磷酸酶及晨尿吡啶啉,尿吡啶啉用肌酐校正。按年龄段(40~49,50~59,60~69,70~79,>80岁)和不同骨密度组(骨质疏松、骨量减低、骨密度正常)对男性和女性的骨代谢生化指标值分别进行分析。结果:中老年男性骨钙素随年龄增长下降,尿吡啶啉/肌酐随年龄增长上升,均低于女性(P<0.05),中老年女性骨钙素和尿吡啶啉/肌酐在50~69岁时明显高于其他年龄组(P<0.05)。70岁以后又趋于下降,尿吡啶啉/肌酐值与同年龄的老年男性相近,差异无显著性意义(P>0.05)。各年龄段男性或女性血清碱性磷酸酶基本在正常范围。中老年男性骨质疏松组和骨量减低组的骨钙素低于骨密度正常组,而尿吡啶啉/肌酐明显高于骨密度正常组(t=7.051,7.898,P<0.05);中老年女性骨质疏松组和骨量减低组的骨钙素和尿吡啶啉/肌酐均明显高于骨密度正常组(t=3.633,17.923,P<0.05);同性别骨质疏松组和骨量减低组的骨钙素和尿吡啶啉/肌酐值比较,差异无显著性意义( AIM:To discuss the characteristics of bone metabolic markers of middle-aged and elderly people along with the change of age,and analyze the value of bone metabolic markers in evaluating the diagnosis of osteoporosis. METHODS:Totally 410 middle-aged and elderly patients,who were treated or hospitalized in the Department of Comprehensive Medicine,Tongji Hospital affiliated to Tongji Medical College of Huazhong University of Science and Technology from October 2002 to March 2004,were involved in the study.The bone mineral density(BMD) of anterior-posterior lumbar spines was tested,and fasting serum bone gla-containing protein(BGP),serum alkaline phosphatase(ALP) and morning urinary pyridinoline(PYD) were measured in all the 410 patients,and urinary PYD was corrected with creatinine.The bone metabolic biochemical markers values of males and females were analyzed statistically among different age groups(40 to 49,50 to 59,60 to 69,70 to 79,above 80-year-old groups) and different BMD groups(osteoporosis group,lower bone mass group,normal BMD group). RESULTS:With the increase of age in middle-aged elderly males,BGP was decreased,urinary PYD/creatinine was increased,which were both lower than those in females(P< 0.05).Female BGP and urinary PYD/creatinine were obviously higher when they were 50 to 69 years old than the other age groups(P< 0.05),and those began to decrease after 70 years old,urinary PYD/creatinine were not significantly different between males and females of the same age(P >0.05).Serum ALP levels in males and females of each age group were in a normal range.Middle-aged and elderly males in the osteoporosis group and lower bone mass group had lower BGP but obviously higher urinary PYD/creatinine than those in the normal BMD group(t =7.051,7.898,P< 0.05).BGP and urinary PYD/creatinine were both markedly higher in the females in the osteoporosis group and lower bone mass group than in the normal BMD group(t=3.633,17.923,P< 0.05).There were no significant differences in BGP and urinary PYD/creatinine between
