摘要
目的探讨应用Amplatzer封堵器治疗先天性心脏病(congenitalheartdisease,CHD)的近期临床疗效。方法51例CHD患者,在经胸彩色超声心动图(transthoracicechocardiography,TTE)及X线影像指导下,经皮沿导丝经传送鞘管将合适的Amplatzer封堵器至缺损处行封堵。结果①TTE测量房间隔缺损为(14.8±7.1)mm,所用封堵器为(19.1±8.4)mm,术后即刻TTE未发现分流,仅1例于术后5h封堵器脱落;②左室造影测量室间隔缺损直径为(6.2±2.9)mm,封堵器(8.1±3.4)mm,无1例存在残余分流,术后即刻完全封堵率100%;③造影测量动脉导管未闭最窄处直径为(6.1±2.8)mm,所选用封堵器肺动脉侧为(10.0±3.2)mm,主动脉侧为(12.0±3.2)mm,术后仅1例少量分流。结论应用国产Amplatzer封堵器经皮介入治疗操作简单、成功率高、安全可靠、近期临床疗效好。
ObjectiveTo evaluate the therapeutic efficiency of Chinese-made Amplatzer occluder device in treating congenital heart diseases(CHD). MethodsFifty-one patients were treated with the occluder devices through the percutaneous procedure under fluoroscopy, angiography and transthoracic echocardiography(TTE). Results① The atrial septal defect(ASD) diameter was ( 14.8± 7.1) mm by TTE. The diameter of the occluder selected was ( 19.1± 8.4)mm. No residual shunts were found in all cases after the procedure. However, one occluder fell off from the ASD after 5 hours of the procedure. ② The ventricular septal defect(VSD) diameter was ( 6.3± 2.9) mm measured by left ventricular angiography and the diameter of occluder selected was ( 8.1± 3.4) mm. The success rate was 100 % and no residual shunts were found in all cases. ③ The narrowest diameter of the patent ductus arteriosus(PDA) was ( 6.1± 2.8) mm by angiography. The diameter of occluder selected was ( 10.0± 3.2) mm in pulmonary artery side and ( 12.0± 3.2) mm in aortic side. Only one patient had small shunts. ConclusionThe operation with occluder devices made by China is simple with a high success rate of placement, a high safety and reliability and good occlusion effect.
