高乌甲素用于人工流产镇痛的临床研究 被引量:1

摘要 目的:比较高乌甲素与芬太尼伍用丙泊酚用于人工流产的临床效果。方法:自愿接受无痛人工流产者120例,ASAⅠ~Ⅱ级,妊娠6~10周,随机分为两组:每组60例,F组(芬太尼1μg/kg),L组(高乌甲素0.08mg/kg),分别观察心率(HR)、血压(BP)、血氧饱和度(Sp02)变化,丙泊酚用量,呼吸频率(RR)及呼吸抑制发生率,恶心、呕吐,苏醒后痛苦记忆发生率及病人术后离院时间。结果:呼吸频率减慢、呼吸抑制及Sp02低于95%者F组明显高于L组,其他观察指标两组间均无显著差异。结论:等效剂量的芬太尼与高乌甲素伍用丙泊酚用于人工流产均可达到满意的麻醉、镇痛效果,但伍用芬太尼时呼吸频率减慢、呼吸抑制发生率相对较高。 Objective: to compare the analgesic effect of fentanyl or lappaconitine plus propofol on artificial abortion. Methods: All of 120 ASA physical status I and II patients scheduled for artificial abortion were randomly divided as 2 groups: group F(fentanyl 1 μg·kg^(-1))and group L(lappaconitine 0.08 mg·kg^(-1)).Each group was 60 cases. The heart rate, mean arteria pressure, saturation of blood oxygen (SpO_(2)),dosage of propofol, the frequency of respiratory decrease and respiratory depression,the frequency of nausea,vomiting and consciousness, etc were recorded. Results: Group F was associated with higher frequency of breath rate decrease,respiratory depression and Sp0_2 lower than 95% than group L.Other indexes showed no significant differences between the two groups. Conclusion: Uniform dosage effect calculated for fentanyl or lappaconitine compatibilitv of propofol applied to artificial abortion could get a satisfactory analgesic and sedative result,but compatibility of Fentanyl ease to cause higher frequency of breath rate decrease and respiratory depression.
