摘要
目的:检测Bcl-2蛋白在不同阶段成人急性白血病的表达分布,分析Bcl-2蛋白表达是否可以作为预测化疗反应的指标。方法:选择成人急性白血病患者骨髓标本72例,分3组:初治组18例,完全缓解(CR)组38例,复发组16例。正常对照组6例。以流式细胞仪和单克隆抗体检测Bcl-2蛋白表达,进行统计学分析。结果:①Bcl-2蛋白在初治组、CR组和复发组表达率分别为11.11%、18.42%、62.50%。复发组表达率显著高于初治组和CR组(均P<0.01),初治组和CR组表达率差异无统计学意义(P>0.05);②Bcl-2在急性髓细胞白血病和急性淋巴细胞白血病表达率分别是24.07%(13/54)和33.33%(6/18);两组差异无统计学意义(P>0.05);③Bcl-2蛋白表达阳性组与阴性组CR率分别是36.84%和77.36%(P<0.01)。结论:Bcl-2蛋白在成人急性白血病复发组高表达并且与CR率低相关。
Objective:To investigate the expression of Bcl-2 protein in adult acute leukemia to see whether it can be used as an index of drug resistance.Method:72 adult acute myeloid leukemia and adult lymphoblastic leukemia patients were divided into three groups: de novo group (18 cases), complete remission group (38 cases) and relapse group (16 cases). the Bcl-2 protein expression was detected by flow cytometry and monoclonal antibody.Result:1.Rates of positive expression of Bcl-2 protein in de novo group, CR group and relapse group were 11.11%, 18.42%, 62.50%, respectively. The expression of Bcl-2 protein was higher in relapse group than that in de novo and CR groups (P= 0.0018, P= 0.0015), there were no difference between the expression of Bcl-2 protein in de novo and CR group (P= 0.49). 2.Rates of expression of Bcl-2 protein in ALL and AML were 22.2% and 27.80%(P= 0.44). 3.Rates of complete remission between positive and negative expression of Bcl-2 protein were 36.84% and 77.36%.Conclusion:Our result demonstrated that Bcl-2 protein was highly expressed in relapse group, and expression of Bcl-2 protein was significantly associated with a lower CR rate.
