摘要 本实验采用裸光纤和金属帽光纤传输连续波Nd:YAG激光,对60条完全性栓塞的犬股动脉血管进行了激光疏通术实验。激光疏通术后分别于1、7、15、30和60天采取血管组织,以光镜、电镜及免疫组化技术观察激光致伤动脉的愈和反应。早期的损伤改变是血管内皮消失,内弹力膜断裂和部分中膜坏死。7~15天时,内皮开始再生并有内膜增生,60天时血管损伤部位完全愈合。内弹力膜断裂后不再连接,但覆盖断裂部位的新生内膜中含有细薄且不连续的弹力纤维,胶原含量也增加。纤维连接蛋白(Fn)仅在血管壁修复区聚集,即在再生内皮细胞之间、增生的内膜及部分损伤的中膜中呈中等到强阳性,与血管壁的愈合密切相关。实验表明,连续波Nd:YAG激光损伤的动脉愈合良好,运用连续波Nd:YAG激光疏通阻塞血管仍然是可行的。 O completely occluded femoral arteries in dogs were recanalized by continued wave Nd:YAG laser withbare fiber or laser-heated probe,30 dogs were killed at 1,7,5,30,and 60 days after laser irradiation,respec-tively.The results of electron and light microscopic examination showed:Early vascular chagges consisted of de-nudation of the intima,disruption of the internal elastic lamina(IEL)and partial necrosis of the media.Reen-dothelialization occurred between 7 and 15 days after laser irradiation,and there was evidence of intimal hyper-plasia persisting up to 60 days.The normally continous IEL,once ruptured,never became rejoined again.How-ever,the neointima covering the defects contained new thin,discontinuous elastic fibers and proliferating colla-gen fibers.The immunohistochemical observation showed that fibronectin(Fn)exclusively accumulates aroundthe endothelial cells,the proliferatin intima and injured media.Fn is closely associated with the healing processof the injured arterial wall.All femoral arteres in experimental dogs recovered well after CW Iaser injury.There-fore,the results of the present study demonstrated that orcluded arteries may be well recanalized by CW Nd YAG Laser.
